Dana Wong is a seventy-year old female who had an ischemic stroke, and is entering our care facility after a seven-day stay at Mercy North Medical Center. The stroke has left her in a wheel chair with decreased mobility on the left side, slurred speech, and relaxed features. The referral came from Dr. Parker on September 12, 2015, which was four days after her admission in the hospital, and request that Mrs. Wong undergo two months of physical, occupational, and speech therapy before she returns to her home at Green Meadows, an assisted care living facility. Dr. Parker noted that Mrs. Wong can benefit from our services, and believes that she can make an adequate recovery, because she is determined to maintain her independent lifestyle. Mrs. Wong has full coverage from Blue Cross Blue Shield, who has agreed to pay for her stay at the facility for a maximum of two months.

Wong’s assessment we began up top with screening her vision and visual perception. We used the Adult Perceptual Screening Test (OT-APST), which showed no initial signs of impairment, but we will continue to recheck her vision, so we can make sure the relaxed features of her face will not produce problems in the future. Next, we tested Mrs. Wong’s memory and cognition, by using the Cognitive assessment of Minnesota. At this time, Mrs. Wong scored a 55, so her cognition is considered mild, and it should improve if she continues working on the memory recall tests. For Mrs. Wong’s upper body, we utilized two tests. First was the Manual Muscle Testing on both the right and left arms. As expected for her left side, most of her muscles scored in the poor range, however, her right side she had difficulty in her pectoralis muscles extending her arm upward and outward. Finally, the grip and pinch strength test showed that her left hand has no strength in it, but her right hand scored

On 12/3, APS SW Jamie Gaines spoke with Rosa Naremore about the possibility of Ms. Mullins receiving home dialysis. According to Ms. Naremore, Ms. Mullins would not have the mental capacity of what needed to be done with home dialysis. Worker explained to Ms. Naremore that Ms. Mullins had recently moved into Eagle Rest and the owner could help Ms. Mullins with the process. Ms. Naremore stated the person willing at assist would be required to attend training at Princeton from 8-12 for a month. In the training, she would learn all the requirements.
Reporter Alison Parker and cameraman Adam Ward were shot during a live interview. On wednesday,August 26,in Moneta,Virginia. The suspect of the shooting was identified as fellow journalist Vester Lee Flanagan II. He appeared on WDBJ-Tv as Bryce Williams, he was fired from the station after a year of working there. The women being interviewed,Vicki Gardner, was shot in the back and as in surgery.
As Lawyer Farrington said, Lou Dempsey was accused of illegally selling alcohol to the minor, Eric Howe. Mr. Dempsey failed to meet his responsibilities of asking for identification and anticipating violence and/or accidents caused from alcohol. Her goal is to gain recovery so she can walk on her own again and eventually move back into her house to help take care of her husband, whom has dementia. She is willing to modify her lifestyle accordingly if she is unable to gain complete recovery. However, she would like to be as independent as possible.
Tara Jones is a 16-year-old female who has been mandated by Alberdene County Court to Smith Center for Youth, residential facility, after running away from her current foster care placement with Ms. Judy Boot. Tara has come in involuntarily stating that she does not want to live in a residential facility and would rather hold residence with her maternal aunt, Sally Johnson. Tara feels that her behavior “is no big deal.” Smith Center for Youth will be closing and Tara will need to be transferred over to another residential facility, Tanner House. Her family therapist, Ida Dogoode, has behavioral concerns Tara that are present that lead to a questionable successful transfer.

This decision of the court was made in a way of jumping straight to conclusions. Was there reasonable doubt that Adnan Syed didn’t murder Hae Min Lee? There was plenty of reasonable doubt, but Adnan still was charged with 1st degree murder.

This court case is about Brandon Moon who got accused falsely on three different counts of aggravated sexual assault. The case is a confusing one, saying that nothing adds up with the couple different sides of this particular case. On one side, where you have Brandon’s girlfriend, with Brandon stating that he was talking with her, and was going to meet up with her. But, on the other hand there were three women that said Brandon Moon was their attacker, but in the end, I guess that the four witnesses beats two of them.

You patient, Jerry, is a 67 year old male who recently had a cerebral vascular attack (CVA) affecting the right side of his body. You have been working with Jerry for the past six weeks on strength, ROM, balance, and coordination and he has been progressively gaining more function. In the past few sessions however, you have noticed that he has plateaued. The insurance company needs to see that the patient’s status is improving in order to continue paying for their therapy. It is noted that studies have shown that it is normal for a patient to see a plateau in the middle of their therapy timeframe, but if therapy is continued a positive progression can be seen; conversely if therapy is stopped then a loss of gains and function can occur.
As the assessment begins for Mrs. Adams there are many things that need to be addressed. Educating the patient on active participation in her healing will be of extreme importance. The interventions are only as good as the patient’s willingness to comply. We would need to begin with gaining control of the cleanliness of the apartment and that of Mrs. Adams. Opening the windows and allowing the sunlight to enter is important along with attempts at fixing the air conditioner.
