Daviess County is the community chosen for this community health assignment. Daviess County is located in the southwest region of Indiana. Several towns comprise Daviess County such as: Washington, Montgomery, Plainville, Elnora, Cannelburg, Alfordsville, Raglesville and Odon. Washington is the county seat of Daviess County, Indiana. Daviess County is located along the White River and has 6.2 square miles of water and land area of 431 square miles. Daviess County was named after the US District Attorney for Kentucky, Joseph Hamilton Daviess, in 1817 (Daviess County, Indiana (n.d.). As of July 1, 2014, the population of Daviess County was 31,654. The median household income is $47,104 with 13.8% of the county population living in poverty as calculated during the years 2010-2014. The cultural diversity of the community consist of 92.7% White, 1% African American, 0.4% American Indian, 0.7% Asian, 0.1% Native Hawaiian, 0.9% Two or More Races and 4.6% Hispanic or Latino descent. The Women, Infant and Children (WIC) target nutritionally at risk, low income, pregnant, postpartum women, infants and children under 5 years old. They offer vouchers for healthy foods such as milk, eggs, fruits, vegetables, peanut butter, canned or dry bean and peas and whole bread. Pamphlet, other health agency referrals and word of mouth is the main advertisement. Senior and Family Services offers an array of services in their facility from adult day care, bingo, bible study, fitness, crafts and so much more. They also provide in-home service such as homemaker services, attendant care service and respite services through the CHOICE program or private pay. Meals on Wheels provides hot nutritious meals once per day to those unable to prepare their meal or unable to shop for food. The agency charges for the meals but take donations for those unable to pay. Both the police and fire department aid in keeping the population of Washington safe. The fire departments speaks at schools and business regarding fire safety and exit plans. The police department also speaks to the schools to prevent drug use with the program DARE. The objective of this project is to improve the Springfield region’s financial performance for the Saints Health. The Saints Health Region (SHR) is accountable for health care services for the 28-county Springfield Arizona Service Area. SHR Health Plan provides operating funds for SHR, and the services are provided through SHR Medical group, local physicians, and other providers. Currently, the operating margin is negative with an operating loss of $110.31 M although the membership growth is positive. As consultants, our team strives to develop strategies to improve the financial performance of SHR by focusing on short-term and long-term viability.
Health Impact Assessment-Provides advice to communities on how they can stay healthy- by evaluating a potential plan, project or policy before it is “built or implemented”(CDC.2015). For example: Maryland state code requires fire sprinklers to be installed in new and renovated single-family homes. Statistics show an 80% decrease in the chance of death in homes equipped with sprinklers. This regulation would also be considered Health in All Policies(HiAP) due to its cross sectoral nature.
These include making more jobs, increasing wages for existing jobs and, offering more jobs with a living wage. For this county to see further growth, it must put an emphasis on discerning new ways to pull more people into the populace through jobs, attractions, and other factors of that influence livelihood. As a largely rural county, this means Kittitas County must look at the positive aspects of other rural areas in the Pacific Northwest as well as throughout the nation to find which characteristics can also apply to this county, and how to incorporate those aspects effectively. More research is also needed for more concrete health data, census numbers, and understanding of which areas the county will need to focus on in the future, following its work to provide more accessible and affordable quality healthcare to the population. Nevertheless, Kittitas County has discovered some areas of needed improvement in the county and is working hard to see the results of these efforts.

Care Standards Act 2000 The Care Standards was established in 2000, its’ aim is to ensure that the standards of care within all institutions were not inadequate as the rules and regulations have to be adhered to. The care standards act try’s to make sure that all institutions are equipped and well facilitated to meet the needs of those within the provision.
Unit 4 is the preparation of treatment in which the baseline data (activity level) is gathered. This includes daily activity record and behaviour contract. A daily activity record ( hour by hour ) is used to monitor activities clients is already doing, to see the frequency of healthy and unhealthy behaviours. (Lejuez, 2001, p. 265). In this phase, the client is encouraged to create an environment, one that promotes healthy behaviours (Lejuez, 2001).
I thought Meals on Wheels of America was just a simple organization covering the surrounding counties that provided a meal a day to senior citizens who had trouble providing a meal for themselves. However, I have learned that Meals on Wheels of America was established in 1954 and is the oldest and largest national organization representing community based senior nutrition programs and that it has 5,000 member programs, in which they provide over 1 million meals to seniors every day, which are served at senior centers as well as delivering meals directly to the homes of seniors whose mobility is limited. Down syndrome is a condition which extra genetic material slows down the way in which an individual develops, both physically and mentally. As stated by the NHS; each individual with down syndrome is affected differently and can vary, some may need a lot of medical assistance whereas others may lead a normal healthy lifestyle but share similar characteristics. Delayed development: All service users with down syndrome has some degree of learning difficulty and delayed development, however, this varies between each individual. Having down syndrome they may experience some health problems which may effect them in every day life. However yet again each individual is different some may have more health problems than others.
In mental health there are a lot of different roles of different agencies and professionals within the assessments, both of these are for the protection of the service user and service provisions these include mental health services which are set up and run by the NHS organisations as well as some voluntary sectors there are also private companies which are paid for by or commissioned by the NHS. A local GP should be able to recognise a service users symptoms and diagnose the service users symptoms of mental health or psychosis then they should refer the service user to an intervention service were they will then be assessed. These assessments will then be carried out on the service user, which will then provide them with the right services
7 / D.P7: Explain how different procedures maintain health and safety in a selected health or social care setting Maintaining health and safety in health and social care is extremely important to ensure the health, safety and wellbeing of all their service users as well as other individuals service providers may come in contact with in the setting. There are several procedures that help to maintain this health and safety however they can all vary between settings for example, health and safety procedures will be slightly different and more focused on certain areas in hospitals and especially in paediatric ward compared to in drop-in centres where the needs and risk to service users are slightly different. Some of the procedures used in health and social care to maintain health and safety include; infection control and prevention, safe moving and handling of equipment and individuals, food preparation and storage, storage and administration of medication and storage and disposal of hazardous substances.
Evi Hernandez has 18 years of work experience in the field of public health, with 12 of those years working within the non-profit sector at the California Health Collaborative (CHC). His experience in the public health sector has been primarily in the areas of tobacco control, alcohol and other drug prevention, teen pregnancy prevention, youth development, and the prevention and management of chronic diseases. He currently serves as the Director of Program Services for CHC. He is affiliated with various health initiatives and community service/volunteer organizations throughout California.
On 3/17/17 at 1005 hrs, I was dispatched to the PD lobby in reference to harassing phone calls. Upon my arrival, I made contact with the complainant Victoria Wert and her boyfriend, Bradley Palmer. Wert advised Palmer’s ex-girlfriend, Alexus Smith has been texting her since 9/27/16 to 3/17/17, during this time Wert advised she did not text her Smith once, and her text messages are rude and unwanted. Wert advised she did not receive any concerning messages until 3/17/17 at 0930 hrs.
