Advanced Concept 1 – World Wide Poverty
During the interview, Dr. Paul Farmer commented about healthcare infrastructure in the United States and other under-developed countries. He also emphasized about the importance of community-based healthcare when used with institutionalized healthcare. Dr. Farmer served for the United Nations as an envoy, and helped Rwanda and Haiti to build its healthcare infrastructure. He also helped many other countries such as Peru, Mexico and Russia using his expertise.
Though Dr. Farmer mentioned many interesting topics, I would like to first discuss about how important it is for the community-based healthcare services to work together with an institutionalized health care services. According to Maurer and Smith,

Farmer was the difficulty experienced by some patients when seeking care for tuberculosis. Even though Dr. Farmer did not provide specific difficulties experienced by patients. He did mention that there were multiple, unseen obstacles patients had to go through. Dr. Farmer stated that tuberculosis is, in some sense similar to diabetes. However, unlike the diabetes, tuberculosis can be cured, but the road to complete treatment is long and difficult. There are multiple difficulties patients must face when receiving tuberculosis treatment. Such difficulties include financial difficulties, social burden and cultural stigma. In some cases, the treatment itself becomes an obstacle. For instance, Sagbakken, Bjune and Frich have recognized that the Directly Observed Treatment (DOT) is an effective treatment method but also serves as an obstacle for many tuberculosis patients. They have stated in their article, “to avoid experiences of disempowerment and humiliation among patient, treatment and care should be organized in a way that safeguards patient’s right to consent to treatment and that allows patients to negotiate an individualized treatment schedule” (2011, p.1). It is well-known fact that immunization have saved millions of lives, however many people fail to understand how immunization is part of public healthcare plan. Similarly, many individuals are also unaware of the importance of removing healthcare inequality. Two major ways that can help relinquish such inequality is either by reducing the poverty level, or by helping underdeveloped countries to build the healthcare infrastructure. These two methods is closely related to one another. If the poverty level is reduced, more people will be able to afford healthcare, where the overall health of the community will improve. However, in order to reduce the poverty level, the government must first implement an economic program or policy that can provide jobs for the people. In addition, there must be available healthcare resources for the public once the people have more money to spend. These days’ patients can either opt out of treatment or health care options in general because the healthcare system has undergone so much scrutiny for many incidents that still go on, because there’s not a day that goes by without see these drug compensation commercials. Compensation for patients whom have suffered the side effects of drugs that were tested on them with vague explanations of how it would work, and we see human beings die off of such careless inhumane acts. Patients should be constantly reminded of their rights, like how the police read one’s Miranda before they arrested it should be the first thing a care giver makes sure his or her patient knows before they agree to any type of treatment that just
Medicine has changed in ways over the years that one might have never thought twice about having anything like that happen to them. People today have increased their knowledge overall about their health situations and how to treat themselves. Patients are stepping up and making decisions about their healthcare choices each day with physicians. And in this process it has turned out to be so important for people to understand what is truly being done before medical treatment is given. We have talked this semester about informed consent and how important it is that our patients understand the meaning of what they are having done.
The thesis of this chapter states that in certain situations, it is crucial to listen to a medical professional, however, in others, it is very important to listen to yourself and also to do what you feel is right. The author of Complications," Atul Gawane, has written this specific chapter to persuade the reader of his thesis. If the choice you make is incorrect, then it could potentially be a matter of life and death. Atul Gawande gives multiple examples of patients that have made wrong and right decisions to prove his point. He uses the personal anecdotes of four different people, with four decisions to prove his point.

The four spheres of political action in nursing are government, workplace, professional organizations, and community. They are all interrelated and interconnected. The government enacts policies that govern and affect all spheres. Healthcare policies are often made to ensure access, quality, and affordability of care. It determines the scope of nursing practice at a state level.
A major healthcare concern is making medicine available to those who are impoverished and live in underserved areas. I grew up in Gloversville, New York, a medium sized town that was at one point considered the most impoverished city in the state. Growing up in this region has given me understanding of what life is like in underserved areas. I 've witnessed first hand, the difference in opportunity between myself and students coming from a more urbanized background. It is easy to empathize with those who do not receive proper treatment due to demographic disadvantages.
Sir Michael Marmot put into perspective how serious social disparities are and how they influence health not only between countries but also, within countries. The issue is not medical care; in fact the United States spends the most on medical care. You would initially think the country in which spends the most on medical care must have higher health rates. Well, believe it or not, this is actually false, for instance, sixty-two countries had higher maternal health rates than the United States. This displays that a lack of medical care fundamentally is not the conflict, whereas social inequality is.
Therefore all health care professionals are required to provide treatment when patient has given consent voluntarily either consent or not to consent, given full information of the risks benefits and alternatives of the procedure
It may surprise you that, according to the World Health Organization (WHO) and World Bank, at least 400 million people lack access to essential health services. They said, at least 6 percent of people in 37 low and middle income countries are living in poverty because they must spend the money for health. Health care services is the most importance thing that we need because it is not only for improve the health but also through individual behaviour and lifestyle choices such as quitting smoking, eating the nutritious food and living a healthy lifestyles.
Health care payments are the ones that push 100 million people every year to enter into poverty line since they are short of financial protection. Sub Saharan countries in Africa can be considered best examples for these conditions (Anyamele, 2011). Wealth becomes a prominent factor for health as evident from the above example. It is revealed that income and health have a correlation where the changes in one affect the other.
The farmers and the poor always are unable to get the sufficient health care. Secondly, the problem is the imbalance of illness 's prevention between medical resources. For a long time, prevention approaches are not well implemented, cure light-proof and lack of government investment in public health and public health system is weak. This situation seriously limited the capacity of service and did not achieve the desired benefits of disease prevention
Because of these issues, society should develop better strategies to help these people in need to eliminate the growing poverty level through the world. These strategies could include
This is so because, universal access to health will really do good to the world and it is a Necessity in order to reduce the level of discrimination experienced in terms of finances . Universal access to health will ensure that there will be access to equitable quality health care and will also give security to those who are financially incapable at the present to afford quality health care die to their financial status. Hence this will be a difficulty for analyst and professional, as they will then have to bring this matter to the conscious mind of the patients. This could also cause tension and opposition by patients during their treatments. This opposition is called resistance. (Nunberg, NCBI, 1943) Therefore patients and analyst have to come to an agreement to be able to solve the conflict.
