Health promotion is defined as the process of enabling people to increase control over, and to improve, their health (World Health). Essentially it is giving people the knowledge, tools, or access to care to become more healthy people. Healthy People 2020 took the idea of health promotion and made it nation-wide. The Healthy People 2020 initiative gives 10-year national objectives for improving the health of all Americans. These objectives are to be met by the year 2020 (healthy people reference). My chronic illness includes migraines, which are controlled with medication. I have never been hospitalized, and the only surgery was a wisdom tooth extraction. I have no known allergies. Physical activity/Exercise - In recent months, I have made a change to become more physically active. Normally, I walk around 2 miles 4 or 5 times a week. I also try to make it to the gym for strength training 3 times a week. It has been my goal to become more active because of the associated health benefits and for weight loss purposes. My family supports my attempts to become more active. My sister goes to the gym with me, and my mother walks with me. Eating habits - In recent months, I also made a change to start eating healthier. I cut out processed and fatty junk foods and now eat a diet rich in fruits and vegetables. I stopped drinking soft drinks and sweet tea. I drink water but still have coffee in the morning. For each meal, I eat a good source of protein, a complex carbohydrate, and a fruit or vegetable. There are times when I still eat sweets. My family has supported my new diet and has also adopted healthier eating habits. Alcohol/Drugs and Tobacco Use - I do not use tobacco of any kind, but I am around other people who smoke. I do not use recreational drugs and only drink alcohol occasionally. If I drink, it is only once a week and …show more content…
The purpose of doing a health promotion plan is to provide a guide for you to go by. The goal of doing the assessment and plan is to become healthier. The timeline for my plan is to start now, have a checkpoint in one month and again at 3 months, and to end in April. I will perform a formative evaluation at my one month checkpoint. I will perform a summative evaluation at the end in April. My support system for this plan will be my family. For the area of physical activity, I want to maintain what I have been doing. I have been walking 4 to 5 times a week and strength training 3 times weekly. Barriers to this are time constraints and motivation. To overcome the time constraints, I plan to try high intensity interval training which gives the same benefits in a shorter time period. I plan to have my family help keep me motivated. For the area of nutrition, I plan to maintain what I have been doing. I also want to decrease processed sugar and caffeine intake. I will begin by cutting my coffee intake in the mornings and every two weeks will decrease again until no intake. Barriers to eating healthy are cost and changing old habits. To overcome changing habits, I will have my family help me keep check on what I eat. For the area of alcohol/ drug and tobacco use my health promotion is to limit my exposure to secondhand smoke. For sleep/rest, my health promotion is to make

They always show improvement in areas that need work. However, there are some discrepancies that definitely stop the progress that Healthy People has set forth.

The Ottawa Charter, an international agreement reached in 1986 identified five key areas of health promotion that needed improvement. These action areas include building healthy public policy, creating supportive environments, strengthening community action, developing personal skills, and re-orienting health services towards preventing illness and promoting health8. Then in 1994, the Public Health Agency of Canada9 defined population health

I strongly value my health and believe that health is our greatest form of wealth. When one lives a healthy lifestyle, it means more opportunities to explore the world, build families, achieving anything the heart desires and conquering personal goals. The behavior change philosophy fits my personal philosophy of health education the most because it involves goal setting, behavioral contracts and self-monitoring to help foster the modification of an unhealthy habit. The behavior change philosophy is very important because change is a process, not an event. Self-efficacy and motivation are key factors in successful behavior changes.
