The goal of this meeting is to help Patrick identify what is hindering him from moving forward pass homelessness and medication management without alcohol.
• Client's presenting problem or concern:
Patrick has abused alcohol for more than twenty years. Due to his alcohol abuse, the client lost his job and became homeless. He is using his food stamps to buy beer and get cash for marijuana, two months ago the client had a minor stroke and two stents put in his heart.

Thomas USVI he and his twin brother are sixth of ten children. His parents work outside the home his mother a nurse and his father owned a restaurant in Charlotte Amalie, USVI. Patrick reported experiencing significant verbal conflict between his mother and his father. He believes they fought a lot because he and his siblings did not help enough around the house, forcing their parents to be overworked and over-stressed. Patrick thought his father bullied him and he carried anger towards his father that makes him bullied others.

Patrick stated he has no friends or family. When he works, he is there for the money not to make friends. Patrick requested his case manager to be his emergency contact.

While the social worker can empathize with the client and has noticed some similarities in their backgrounds and life experiences, the client showed poor judgment in drinking and using alcohol to lower his stress. The counselor does not believe that counter-transference issues developed during the assessment.
Patrick is a sixty-eight-year-old single, male. He appeared older than his stated age. His dress was appropriate for the setting and meticulous. He was wearing blue jeans with a black belt, white polo shirt, neatly tucked in, and black shoes. Patrick was cooperative in the interview answering all questions politely. His mood was both anxious and defensive. Patrick’s affect was appropriate to content; he was not overly emotional but reasonably distressed over his current situation. His rate of speech was somewhat rapid when addressing stressful subjects, but otherwise healthy. His tone was average throughout the interview. His thought processes were not logical, yet he did not demonstrate proper insight as to his actions, and how they contributed to his situation, and his continued inability to stop drinking alcohol.
