PER REPORTER: On 10-7-2015 Dottie drove to the veterinary clinic with Brileigh in the car. The reporter said that Dottie was so mental disoriented that she could not tell the staff why she was there. The reporter said the staff became concerned about the welfare of Brileigh and was going to call 911 until Dottie’s daughter-in-law came driving by. Per reporter the staff made the daughter-in-law call 911. Dottie was brought to the Wayne General Hospital by EMS. Upon arrival the reporter said Dottie was in and out of conscious. While at the hospital Dottie’s son, Steven stated that Dottie miss uses and abuses her medication. Steven said that Brileigh is supposed to give Dottie her medication. On 10/22/2015 SO EMT Perez was dispatched to FC 418 regaurding a fallen female resident. SO EMT Perez knocked and announce his presence at the door and was greeted by the spouse of the resident who invited him in. The fallen resident in question was on the kitchen floor sitting upright, a Mrs. Barbara Murtishaw. Mrs. Barbara Murtishaw had fallen in her kitchen and needed a lift assist because she could not get up on her own strength. Mrs. Barbara Murtishaw stated that she was not in any pain and only wated to be lifted upright.
On 7/20/2015 S/O EMT Perez was dispatched to MG-601 regarding a fall. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/ O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor.
On 1/17/2016 SO EMT Perez was dispatched to HG-407. SO EMT Perez knocked and announced his presence at the door. SO EMT Perez was greeted at the door by the resident. The Resident, a Mrs. Elsie Cooperman answered the door and was activly bleeding from her face as she was trying to explain what had happened. SO EMT Perez immediatly began to controll the active bleeding and had Mrs. Elsie Cooperman sit down in a nearby chair while he tried to also calm the resident down so he could get some information about what happened.
On 9/25/2015 SO EMT Perez was dispatched to VP-113 regaurding a fall. S/O EMT Perez knocked and announced his presence at the door and was greeted by the residents spouse and introduced to the fallen female resident a, Mrs. Dorothy Gish who was sitting upright on the living room floor by her rollater. Mrs. Dorothy Gish stated that she was sitting on her rollator when she slid onto the floor and was unable to pick herself up on her own strength. S/O EMT Perez performed a pelvic exam on Mrs. Gish which revealed no pain or grimace.
Introduction A divorced, thirty-eight year old, Hispanic woman, named Anna Garcia, died on August 25, 2015 at approximately 11:00 am. Anna’s best friend and co-worker, Lucy Leffingwell had reported to officials that Anna had been in the hospital, experiencing back pain, radiating towards her sides, Lucy also commenting that Anna told Lucy that she had stopped taking antibiotics that were prescribed to her. At about 9:45 am, Anna’s neighbor named Doug Greene contacted the police reporting that he is worried about his neighbor Anna, explaining to officials that he experienced seeing Anna at at approximately 630 am wearing a sweater while that part of town was experiencing a heat wave recently. The neighbor also said that Anna’s dog had been barking excitedly for the last two hours.
R/s Sandra told Anna she was going to call DSS and said Anna couldn’t come back home. R/s Anna recently moved back with her mother due to having multiple surgeries on her mouth. R/s Anna is afraid she doesn’t have anywhere to go this evening. R/s Anna also reported that one her mother mushed her in the head.
The case to be presented involves the details surrounding the death of Mrs. Ursuline Alfred while under the care of the Hollywood Medical Center, now named Memorial Regional Hospital South, on May 18, 1996. Mrs. Ursuline was a forty-five year old female believed to have suffered a seizure in her home at approximately 1930. She was found on the floor by her husband, after shouting “My head”, in a coma-like state. Mr. Alfred activated EMS and the patient was transported to HMC immediately. While in transport, Mrs. Alfred recovered and EMS recorded “near normal” vitals at 0800 per the District Court of Appeals of the State of Florida (2012).

On 7/9/15 worker made an unannounced visit to the residence of Ms. Bernice Connell, for the purpose of making first victim contact. Ms. Kayley McKinnon, granddaughter-in-law of Ms. Connell greeted worker at the door and showed worker to Ms. Connell 's room. The room was cluttered but did not present with an odor. Ms. Connell was lying in bed watching TV, she was appropriately dressed with good personal hygiene. Ms. Connell stated she had lived with her son, Earnest McKinnon, and daughter-in-law, Arlinda McKinnon, for two years.

Mr. Earlington was familiar with Kyree Dobyns and his mother. There were no prior incidents involving this subject. I called Keisha Dobyns (mother) and spoke to her by phone. I confirmed that Kyree was late coming home on October 15, 2015. I asked Keisha Dobyns if she confirmed that her son was in fact at work on October 15, 2015.
I responded to Troy Regional Medical Center in reference to an assault call. Upon arrival, I made contact with Roseline Jackson in room 9. Jackson stated that her and her baby daddy got into a physical altercation around 13:00 on this date. Jackson stated that she was picking their son up from Bean mother house and that’s when Bean asked to borrow her car. Bean got mad because Jackson wouldn’t let him use it.
