1. Accidents These are such common occurrences happening all over the world, at home, school, road, factories, farming, at sea and air, on a minute to minute, and on a daily basis. So what is this accident? An accident can be said to be an unexpected, unplanned, and unintended occurrence which may involve injury to people and damage to properties. A myth surrounding accidents is that they are considered unforeseen, uncontrollable, and, consequently regarded as fate, destiny, misfortune, an act of God, demons chance, and the like, but the truth is accidents are foreseeable and preventable. When accidents do not cause injury or damage to property, it is called an incident or near-miss accident. However, this is also taken into account while reckoning the risks. 4.2.2. GANTT Charts These charts are useful to schedule investigation, depict probable sequence of events, and events that occur simultaneously that lead to an accident. 4.2.3. Fault Tree (Causal Tree) A fault tree is a logic diagram which depicts the circumstances and events that sequentially combine to end in an accident or undesired events. 4.2.4. Analysis Analysis depicts the direct cause, indirect cause, and other causes. 4.2.5. Report This is produced after an investigation is prepared and submitted to the designated authorities. The report should contain background information like where, when the accident happened, the materials and machines used, and persons involved in the accident. One should give a description of the actual accident, the events leading to it, the accident type and the hazards present. 4.2.6. Recommendation This should be given for immediate, short-term, and long-term prevention and control of direct, indirect, and underlying or basic cause that should be clearly mentioned. 5.
