Although they have defined hierarchy and approval is needed at every end but as compared to traditional organizations culture of advertising agency is different and more based on creativity. Like promotions here are based on how skillful a person is.

If client and you agency’s relationship goes sour then what do you guys do? Do you guys follow 90 days policy of cancellation?
As advertising agency is a service provider so they never let relationship sour and they always find a way to carry it out. Cancellation is obviously possible but there are constrains about price and most important long term relationship.
How do you deal with small budget projects and still come up with effective campaign?
Even if the budget is big enough advertising agency try to complete it in as low as possible. Moreover, they are advisors so they guide client about the best possible medium to advertise depending upon the type of business.

Freedom Summer, or the Mississippi Summer Project, was a volunteer campaign launched in June 1964 to attempt to register as many African-American voters as possible in Mississippi, which had historically excluded most blacks from voting. The project also set up dozens of Freedom Schools throughout Mississippi to aid the local black population. The projects leadership and funding came from the SNCC and COFO, along with hundreds of white college students in the north. In 1963, the SNCC organized a mock vote for blacks, which gave them a chance to prove they were capable of understanding politics.
