Say no to Sugar Daddy
Introduction
The mortality and morbidity rate is high in Swaziland due to HIV and AIDS epidemic, however, one of the drivers of this epidemic is the intergenerational sex, a sexual intercourse between an older man, popularly known as ‘sugar daddy’ and the Teen, who is still in the primary or secondary school. The objective of this national campaign, ‘Say no to Sugar Daddy,” was to raise awareness among Teens on the consequences of sexual intercourse between an older man and a young girl, which have been the driving force of high HIV incidence and Teen pregnancy among young girls. This campaign was an idea for a single woman, who shared her vision with other women, and the fire ignite. I learn that it does not take a whole community to change the community, but one man with a vision can bring transformation to the community as a whole. The preconceived idea I had was that an individual cannot make a difference unless a group of people comes together and agree, and then maybe transformation could come. This is now in the past now, this campaign has helped me to dispel those notions, now I know that one person with a vision can change the course of history. An article by Bahr discusses importance of community outreach and involvement in the treatment of heart attack patients (Bahr, 2017). Bahr found heart attacks have beginnings which can be identified and acted upon at the community level before reaching the hospital (Bahr, 2017). Bahr’s article describes a community intervention in the past which improved heart attack survival rates by sixty-two percent by increasing bystander and community participation (Bahr, 2017). A different evaluation by Lachance showed programs which emphasized self-participation and health literacy by utilizing community health workers showed positive patient outcomes (Lachance, 2017). Utilizing community health workers in the Hmong population could be beneficial population as they suffer from being stuck in low-wage jobs (Fadiman, 2012, p. 206).
It suggested that people have control over their own health, such as having the choice to smoke or not, and disregarded the social determinants that may lead an individual to take up bad habits7. Regardless, using the health promotion framework during the 1980’s the Canadian Institute for Advanced Research was able to produce evidence of the health inequities present among Canadian populations that contributed to the push for policy changes to address these issues7. The Ottawa Charter, an international agreement reached in 1986 identified five key areas of health promotion that needed improvement. These action areas include building healthy public policy, creating supportive environments, strengthening community action, developing personal skills, and re-orienting health services towards preventing illness and promoting health8. The United States is currently integrating the population health framework into its health care system to be understand the different determinants of health. As described by Jonas & Kovner, population health involves primary prevention, as well as the ability to involve social, behavioral, and environmental determinants of health in a way that the patients will be able to carry out their medical providers’ recommendations about lifestyle behaviors to reduce potential complications as well as to prevent social crises such as homelessness and losing jobs (95).

The lack of sexual education also contributes to young marriage and motherhood rates (Ngo & Leet-Otley, 2011). Hmong parents do not understand sexual education and do not discuss it with their children. In Meschke and Dettmer’s (2013) study on sexual health communication, Hmong daughters inferred that parents believed the talk about sexual health might encourage their daughters to be sexually active. Talking about sex is impolite and embarrassing so Hmong families do not usually have discussions on that topic. However, when they do give out information it is often wrong and exaggerated (Meschke & Dettmer, 2012).
Health promotion and health education have many similarities, despite them being distinctly different. Health promotion and health education are both very important, especially when being used as tools in the hands of different authority figures and policy makers. Health promotion and health education both advocate for health in a systematic way or as a planned application, however we learned in class that anyone can promote health, but not everyone can be a health educator when it comes to health education. Health promotion entails methods by enabling individuals to gain control over the determinants of their overall health. The prerequisites of health are addressed by developing healthy public policies.

Social problems include parental guidance, access to government grant, peer pressure, substance abuse, poverty, job marketing and education. Most teenagers do not have enough information on sexuality and contraceptives. Teenagers engaging in unprotected sexual intercourse lead to unwanted teenage pregnancy which is a serious social and health problem. About half of all South African teenagers aged between 15 and 19 reported having had sex. Teenage pregnancy is considered as one major distraction to the success of many girls’ education.
