On 8/14/17 Joseph Gomez, Wellness Coach (WC) made a visit to Angel Lopez (Tenant) at his apartment for the purpose of following up on her mental health, medical wellness and to address any issues she may be experiencing. When WC arrived Tenant was quite cordial.
Tenant was dressed appropriately and was well groomed. When WC arrived tenant was reading the news on the internet. He appeared to be mentally stable.
SYMPTOM MANAGEMENT
WC asked Tenant how he was doing and he responded he is doing fine and has no pressing issues or concerns to report. He reports he has no money management problems or depression bouts.
Tenant continues to have difficulties with his snap on dentures. snap-on dentures fit over any remaining teeth and attach

Tenant reported that recently had a colonoscopy that allows his doctor to look at the inner lining of his large intestine (rectum and colon). WC encouraged Tenant to keep all medical appointments and speak to the physician about any concerns he may have.
Tenant then told WC a bothersome account of “friend” that asked him to give him a $100 personal loan. A week late tenant asked this “friend for a favor. He needed a colonoscopy and needed someone to accompany him to the appointment. He then asked the “friend” to accompany him and the ‘friend” agreed to go with him
A week later, tenant asked friend when he would be able to repay him his $100 loan? The friend bluntly refused to pay him back his money. The “friend” replied that he would not pay him back because he escorted him to the medical appointment.
Tenant was counseled about the dangers of lending out money to so called friends. He was also warned to be careful around some friends that may want to take advantage of him. Phillip Perez was incensed when he walked in the small apartment. He made his presence known by slamming the door open with an immense strength fueled by drunken rage. He has received some upsetting news that is the cause of this inappropriate entrance on this late night. His girlfriend, Melanie Krizek is flirting with another man according to a friend he was with at the club earlier. His unreasonable response to this rumor is to loudly enter the small apartment and wake the two ladies occupying the home.
The counselor called an ambulance to rush Sanchez to the hospital because she needed an immediate psychiatric evaluation. The hospital employed a standard for admission to its psych unit that boiled down to a simple question: do you feel suicidal or homicidal? To that, she responded “no”. Just 11 minutes after her psych evaluation ender, she was discharged from the hospital with the name of a clinic she could contact for outpatient services, though she was given no address or contact information. She never made an appointment.

Vicente Flores is a twelve-year-old, Hispanic, male. In class, he is one grade level below grade level and is an English Language Learner. Vicente understands and speaks conversational English with hesitancy and difficulty. He understands parts of lessons and simple directions. He is also a pre-emergent or emergent level of reading and writing in English.
The patient is a 52 year old female who presented to the ED via EMS with bizarre behaviors. Per documentation neighbors found the patient screaming in her house. Per documentation LEO found the patient attempting to drink a closed bottle of alcohol hand sanitizer fluid. Patient presents with disorganized thoughts and irrelevant subject matter when asked questions about behavior upon arrival. Nursing staff was asked about status before the assessment and reports improvements in the patient bizarre behavior.
This patient was not treated with the ethical respectany patient should receive when seeking help/treatment. It is very alarming that a physician whose job is to take care of other humans would disregard giving a proper
US v. Lopez was a decision handed down by the US Supreme Court in 1995. The case was significant because it was the first ruling to set limits on Congress's power under the Commerce Claus in the Constitution since Franklin D. Roosevelt's New Deal. As the assessment begins for Mrs. Adams there are many things that need to be addressed. Educating the patient on active participation in her healing will be of extreme importance. The interventions are only as good as the patient’s willingness to comply. We would need to begin with gaining control of the cleanliness of the apartment and that of Mrs. Adams. Opening the windows and allowing the sunlight to enter is important along with attempts at fixing the air conditioner.
