The role of the APRN in the treatment of insomnia in older adults is multifaceted. As providers, APRN’s are skilled in assessment, diagnosis, and management of acute and chronic conditions. The care of older adults with chronic insomnia require a unique approach to management, as first-line treatment advocated in this paper entails a cognitive-behavioral approach that is not routinely utilized. Thus, the APRN holds as a provider requiring specialized assessment skills previously discussed to ensure the proper diagnosis and treatment. The first session will focus on explaining the primary treatment components of CBT including: (a) stimulus control, (b) sleep restriction, (c) cognitive therapy, (d) relaxation techniques, and (e) sleep hygiene. Sleep education regarding sleep norms, circadian rhythm, effects of aging on sleep, and sleep deprivation will be discussed as the cognitive element of therapy. Factors related to stimulus control and sleep restriction will be discussed and patients will develop their standard sleep and wake times. The time in bed (TIB) prescription determined by baseline sleep log data obtained prior to therapy which will measure average total sleep time (ATST). The prescription is based off of the formula TIB = ATST + 30 minutes. Patients will be re-educated on the use of sleep diaries as well. Session two will focus on sleep hygiene education and sleep logs will be reviewed and necessary titrations made. Sleep Efficiency (SE) ≥ 90% will increase bedtime by 15-30 minutes. SE between 85-89% will warrant no change, and SE < 85% decrease bedtime decrease by 15-30 mins. Consecutive sessions will include the sleep log review and titration. Session three will provide education regarding relaxation techniques including diaphragmatic breathing, progressive muscle relaxation, and visual imagery. Twenty-six families with children aged 8–17 years participated. The study compared a treatment and a wait-list control group at baseline, post-treatment and 2 months post-treatment on measures of child and parent functioning. Results demonstrated that the Sleepwise approach was effective in reducing sleep disturbance and parent stress.
