The reporting party (RP) stated there was a bed bug infestation in the facility two weeks ago and resident Joel Lopez-Corrujedo Jr. had bed bug bites on his arms which resulted in a visit to the doctor. His treatment included a prescription for ointment. In addition the RP states there is no hot water in the facility due to the hot water heater being broken since 11/17/16. According to the RP the minor residence are required to use the recreation center nightly to shower. There is a Case Management - SIR - Water Heater reported dated 11/30/16 from the Administrator Inez Wright who notified Community Care Licensing of the broken water heater and contacting a plumber (Right Way Plumbing) and the difficulties receiving the necessary part to fix
CCIB LPA Perryman-French received a call from Mildren. her husband Julian lives in this facility. He is non-ambulatory and requires assistance to utilize the bathroom. Mildred stated that when staff call in, the director does not replace them or cover behind them, the result is that her husband cannot get the assistance he needs to use the bathroom. This has increased his accidents.
Fred reports that on April 22 at 11:00 am, he had a meeting with Kelly Washer from Kingston Property Standards due a complaint from a tenant at 131 Notch Hill, Unit 105. Kelly Washer found no merit to the tenant’s complaints and only found two minor issues that even she is unsure that these are issues at all. One missing cover plate on back of bathroom medicine cabinet and a loose electrical outlet for the stove.

The referrals were not received or processed, however you completed a Case Management Visit on 4/1/16 regarding information contained on the referrals. How would you like me to handle the referrals. CCIB Intake received referral #0923-2901-5102-8047131 from reporting party (RP) Katie Hacker Sierra Child and Family Services staff member. According to the RP child Shawn Williams age 10, informed a group home staff member (name not provided) on the morning of 9/26/16 that during the night his roommate Arthur Hammett age12 had touched him in his "inappropriate parts." Shawn failed to provide any specifics regarding the incident and the RP didn 't question him due to his frequent fabrications as of late. To the RP knowledge, Arthur has not displayed this type behavior before, however Shawn has a history of sexualized behaviors which has been previously reported to CPS.

Reporting party (RP) stated that after visiting with her client, resident Bruce Thompson (DOB: 06/24/60), RP removed the resident from the facility due to severe bed bug sores that were bleeding. RP stated that she has not seen a case of bed bug bites this severe. RP stated that they left the resident's belongs at the facility along with his medication. RP stated that the caregiver/facility (Caregiver information is unknown to RP) would not relinquish the resident's medications to him. RP stated that the resident paid $1,000.00 per month to resident at the facility.
On 08/28/15 Steven came to school covered in live bed bugs and bed bug bites; this is the second time this has occurred. Steven told the reporter his grandmother treated the bed bugs on Monday but there is still an infestation and he wakes up at night with bugs on him. The family is poor and in need of resources but the child’s basic needs are met. Steven doesn’t require any medical treatment. Kevin also told the reporter his grandmother yelled and screamed at him the last time the school called her about the bed bugs.
The licensee 's replied she was removing the furniture to have the home fumigated. On 8/5/15 worker visited the residence of Ms. Mona Watts, for the purpose of assessing her need for services and making first victim contact. Upon entering, the home worker noticed what appeared to be sewage under the trailer beside the steps. The home smelt like the powder off a dog flea and tick collar. There as a dog in the home and according to Ms. Watts, the dog is elderly and unable to use the bathroom outside and that was the reason there was dog urine and feces in the floor. the dog did not appear to be an elderly dog.

CCIB received a SCAR/CPS referral #0306-4829-5973-3020047. The reporting party (RP) Ashley Shinn reports her son, Aedan Shinn (DOB: 01/10/12), attends the Reyes family Day Care Monday thru Friday. RP reports a couple of months ago, her son attempted to tell her how the licensee disciplines the children when they are not asleep during their nap time. RP states Aedan at first was going to tell her, but then said he would get in trouble for being bad. As recently as yesterday, Aedan finally disclosed to the RP that licensee when checking in on the children taking a nap and notices the children not sleep with their eyes closed she will hit them on top of the head with a closed fist.
On 6/19/2015 client attended to her ILP meeting. Client was very upset stated nobody is helping her to obtain housing. Cm reminded client that she must provide paper work to HS in order to help her. Housing: HS reminded client that three weeks ago she required her to provide her medical history, a psychosocial, her daughters pay stubs and her daughter must add herself in client pay case.
