On 7/19/2015 SO EMT Perez was dispatched to CC-501 regarding a re-bandaging of a pre-existing room. S/O EMT Perez knocked and announced his presence at the door and was verbally invited in by the resident. Mrs. Betty Robb stated that the bandage that she had over a skin tear on her right forearm had fallen off and would like for it to be re-bandage. S/O EMT Perez provided re-bandaging for Mrs. Robb. CCIB LPA Perryman-French received a call from Mildren. her husband Julian lives in this facility. He is non-ambulatory and requires assistance to utilize the bathroom. Mildred stated that when staff call in, the director does not replace them or cover behind them, the result is that her husband cannot get the assistance he needs to use the bathroom. This has increased his accidents.
On 1/20/2015 SO EMT Perez was dispatched to KC-304 regarding skin pain. SO EMT Perez knocked, announced his presence and was verbally invited in by the resident. The resident, a Mrs. Marian Fox stated that she has been having some severe skin irritation the last couple of hours and she wanted a second opinion on what she should do. SO EMT Perez performed an assesment which revealed the following; Blood Pressure 120/80, Pulse 64, no visible lesions or abrasions of the area where Mrs. Fox was complaining of pain and no other pain outside the ordinary. Mrs. Fox than decided to lay back in her bed.
On 8/1/2015 S/O EMT Perez was dispatched to FC-609 regarding a fall. S/O EMT Perez announced hispresnece and knocked at the door and was verbally greeted in by the resident. The fallen resident, a Mrs. Ida Looney stated she lost her balance and fell while trying to get up from the living room couch and was unable to get up on her own strength, Mrs. Looney was on the couch by the time S/O EMT Perez arrived. Mrs. Looney stated that; she was unaware of any change in medications nor was she aware that she was on any blood thinners, She did recall and remember the fall and was unaware and or could not recolect weather or not she was seeing a doctor here at riderwood. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/ O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor.
On 1/17/2016 SO EMT Perez was dispatched to HG-407. SO EMT Perez knocked and announced his presence at the door. SO EMT Perez was greeted at the door by the resident. The Resident, a Mrs. Elsie Cooperman answered the door and was activly bleeding from her face as she was trying to explain what had happened. SO EMT Perez immediatly began to controll the active bleeding and had Mrs. Elsie Cooperman sit down in a nearby chair while he tried to also calm the resident down so he could get some information about what happened.
CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. On Saturday 11/14/2015 at 2322 hours, Security Supervisor Steven Evans along with Officer Omar Alonso responded to a call for assistance at the Results Waiting area of the Emergency Department. Upon arrival at 2323 hours we were told by a Patient Tech that Nurse Lisette Van Windt had already taken the patient to room # 59. It was explained to Security that the patient, Brenda Pemberton (FIN #84838930) was arguing in a loud manner with her husband and upsetting the other patients in the results waiting area. Security staff reported to room # 59 and were told to wait outside while Nurse Van Windt spoke to the patient about her behavior. At 2134 hours, Registered Nurse Van Windt stepped outside of the room and stated that the situation was under
CCIB received SOC 341 involving resident Camille Griffith (DOB: 03/09/84). Resident left a message with North Los Angeles County Regional Center (NLACRC) supervisor stating that her roommate, Patricia raped her. The resident later stated that it was consentual and then changed her statement to not consentual. Last week, the resident stated that she engaged in consentual sex. The story as reported by resident is that she slept with her roommate 's boyfriend.
On the date of 03/08/2017 at about () hours I was dispatched to the address of 300 N 6th St (the Greybull Motel) in Greybull to a reported domestic violance situation. I was informed by dispatch that the reporting party was in the office of the motel at this time. Upon my arriavl at the above location I spoke with the reporting party, Donald MOFETT() who informed me that his girlfriend had struck him multiple times in the face, neck and shoulder. I observed that MOFETT 'S left eye apeared to be swollen, I also observed marks on the left side of his face, and on his right sholder. When I asked MOFETT what had happened prior to the incident he stated that himself and his girlfriend Montica ROMERO() had gotten a motel room to have some time away from her family that they are staying with, and that both parties had consumed alcohol.
R/Sgt reviewed the complainant’s allegations with her. The complainant, Michelle BLACKWELL related that she had put the incident behind her and did not wish to discuss the allegations with the R/Sgt. Prior to our arrival, Sergeant Gomez #3196 arrived on scene and advised that there was a female, victim of a possible gunshot in front of 101 W. Then, we continued ventilating the patient and placed him on a backboard in preparation to transport and move the patient.

The family members of the patient Patricia N. Lee 75/F located in room 524 are in a dispute over who can see the patient. Both parties have restraining orders against each other. The daughter Lynda Lee has a restraining order against the boyfriend Robert Rolon who is the significant other of her sister Christa Lee Allender. Both parties are saying they have the Power Of Attorney for the patient. The Charge Nurse Colleen Gibbon separated the parties into two different areas.
