CCIB received a telephone call with an allegation against the facility. RP called to report that a program participant of the Resource Center Candice McGurre (435-899-9390) called to stated that her children that attends this daycare told her that Ruth has slapped children on the hand and also shoved a child against the wall. RP did not have any more details of child(ren)names or when these incidents occurred.
CCIB Intake received a cross report call from Marisa Romero with the County of San Diego CPS. Marisa stated she met with Detective Elizabeth Hundley to initiate the investigation of Gabriel Munoz DOB: 4/6/15 head injury. Marisa stated she and the Detective have statements from teachers regarding the incident. The Director was also interviewed and denied the incident happened. The Director continued to deny the incident even when the teacher Monika Huynh admitted the child hit his head when she fell.
CCIB Intake received a SOC 341 regarding an incident which occurred on 5/12/16 involving client Christian (last name unknown) and client J.W. According to the reporting party (RP) Program Manager Jim Kaing at 3:10PM while Christian was in the kitchen eating a snack he suddenly rose from the kitchen table and walked into the "Art Room" and pule the hair of client J.W. According to the RP Christian pulled the hair with two hands resulting in some hair loss and a pin sized blood spot on the her scalp. Brenda prompted Christian to have seat and the RP requested Christian to apologized to client J.W (which is consistent with his mother 's request due to his understanding of the nature of his behavior). Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
On 3/29/2016 I spoke with Leahanne Garcia at the operation. Leahanne stated she has been employed at the operation since May 2015 as a Teacher assistant. Leahanne stated on Wednesday (3/23) that the children had just woken up from nap and Andi was leading circle time, while she was moving the block center shelf back to its position. She stated the children had started to get up from the rug for center time when she heard Andi yelling "get up, get up" and was yanking Isiah up by his arm. Andi then started telling him "you will listen to me" while pointing a finger in his face.
On 6/29/15, Ms. Wendy Sanders and Mr. Charles Ray Hamilton visited the DHR office. Ms. Sanders is interested in becoming Mr. Hamilton 's rep. payee and caregiver. According to Ms. Sanders, she needs somewhere to live because she recently moved to the Carbon Hill area after getting out of an abusive relationship. She has been cleaning Mr. Hamilton 's home getting it ready for her son who is 13 years old. Worker explained to Ms. Sanders how his grandchildren have caused problems in the past for people who have tried to help Mr. Hamilton.

The worker contacted Crystal Thorton; who is a friend of Chetara Washington. The worker asked Mrs. Thorton if she had any concerns for Mrs. Washington and/or family and she stated “No Chetara is a very good mother and a very capable parent. She very patient and you have to be with two children so close in age, but I’ve never seen her yell or curse at her children. Chetara will just have sit in time out or send them to their room if they’re misbehaving. The worker asked Mrs. Thorton if Mrs. Washington had anyone other than her kids living in the home and Mrs. Thorton stated “No.
Assistant District Attorney John Norsetter concealed exculpatory evidence for close 11 years. Ralph Armstrong was convicted for rape and murder in 1995. John Norsetter was aware that Armstrong was not guilty of such tarrying crime, however, he remained silence even though he knew Armstrong was innocent. Back in 1995, a woman made a call to the district attorney testifying that Stephen Armstrong, who was Ralph’s cousin was the responsible of the crime. The prosecutor never told defense attorneys of such valuable information.

Per the SOC341 on 8/13/15 resident Mullin Bonifacio 14 DOB: 3/23/01 used his foot to fondle 19 year old resident Jonathan Winder 's genitals under a blanket while watching television. According to the report Jonathan 's diagnosed with Oppositional Defiant Disorder (ODD) and other undiagnosed developmental disorders. The report revealed that both residents engaged in the incident mutual and consensually. Resident Mullin is diagnosed with bipolar disorder and intermittent explosive disorder as well as pervasive developmental disorder. According to the report both residents have been separated however they continue to reside in the group
On June 18,2015 A.Harper and J. Brown was out of their room and were non-compliant to go back in. The incident occured around 11:30pm. Carl Jay came up to the third floor and told them to go back to their rooms once he left Brianna Hooper went into A. Buch’s room. When asked to go back to their room she hid in A. Buch’s bathroom. A. Mahogany is very aggressive verbally. Consequently, Felecia asked Mahogany to leave her home. The manager (unknown) of the apartment complex contacted law enforcement.
