On 02/25/2016 at approximately 04:32 hours I was dispatched and responded to the docks to the rear of the seaside inn located at 5330 Treadway Avenue. in the City of Port Richey. Upon arrival I made contact with the complainant/witness Daniel Guess. Daniel stated that he was in the cabin of his boat which was docked in an adjacent slip behind the Seaside Inn. Daniel indicated that the boat ("Paladin") next to his collided with his boat.
Regulations, Standards, and Ordinances The Vet Center is a rental property that is being use by their employees to conduct their daily operations. The facility falls under Charleston VA Medical Center and must adhere to their Standard Operation Procedure (SOP) and local ordinances. The VA Medical Center security inspects the property on an annually basis. If requested, security personnel will visit the site for any issues or problems. RECOMMENDATION: The facility does not have any written regulation posted or available on site other than their SOP.

The physician has written discharge order for Rudd. Rudd`s son, Matthew is also at the bedside, waiting for the nurse to bring the discharge paperwork. Rudd`s blood work and X-ray, CT scan results do not show any signs of organ damage. The assigned RN checked vital signs before discharge. The vitals are as follows:

I enter Elizabethtown, just across the river from Gettysburg. Gettysburg was a time bomb waiting to explode because of the closeness of the Confederate army stations circulating this place. The trauma doctors navigated throughout the whole station giving instructions, suggestions, and orders to empty hands like me. I was occupated with a lot of bleeding out soldiers and deaths were even smelled when you walked in. I helped the most I could, but my training wasn't enough to save these soldiers
The event that took place at Nightingale Memorial Hospital was child abduction. In this case the child was dropped off for an operating room procedure and the mother was told it should take approximately one hour and forty-five minutes. The mother instructed the pre-op nurse that she had an errand to run with her oldest child but would return before the surgery was completed. The mother gave the pre-op nurse her phone number to be reached if the surgery was quicker than expected. One Tina was finished with surgery and ready for discharge she was discharged to the care of her father.
