Ms. Cardiello is a 40 year old individual, who was born in Manhattan and raised in Franklin Square. She graduated from a joint high school, Carey and Sewanhaka high school in 1992. She continued her education at Nassau Community College where she obtained an Associate in Liberal Arts. She indicated following her degree from Nassau she received an academic scholarship to Dowling College where she graduated in 1996 with a degree in psychology and a teaching certification. She indicated that she has not had a stable work because she has been in court for many years and it has affected her ability to work full time. Ms. Cardiello reported that currently working full time at a parochial school as a kindergarten teacher.
The book Monique and The Mango Rains is a book that is focused on the friendship between the author, Kris Holloway and Monique Dembele. Monique is a midwife in Nampossela, Mali; she has focused her life on making childbirth easier on many of the women in her community. Due to Mali being in terrible conditions birth became a life of death situation when death was predominately winning. Kris Holloway was working for the Peace Corps and became Monique “assistant”, because they worked together for two years they managed to grow a very genuine relationship. Kris job wasn 't exactly the same as Monique’s, according to her she worked more with the surroundings other than the patient itself. She was focused on educate the families on healthier lives, help converse fuel, fix water supply… so on and so forth. For the reason that Monique was the only one qualified to preform this type of work with this amount of quality they decided to work together and help and make Nampossela’s health care better. Monique is mostly focused on pre birth visits, the actual birth of the baby, post birth visits, keep track of medical records and vaccinations, and treat a couple of medical complications that can come along the way after post birth, for the mom as well as the child. The Board President at the time, Mark Mulpeter, recommended Jackie to the organization and the agency could not have gained a finer literacy advocate and friend.
The sixties have been recalled many times as a revolutionizing era, where America gained Her strength and Her individualism. The sixties were a time of self-expression and complex change for the United States of America. This period of change was very prevalent in many American lives. Sandra Cribb, a very close family friend, recalled what the sixties (1954-1975) met to her, and how this change in culture, politics, and technology, related to her life.
To begin the meeting, Chris provided each attendee with a copy of a calendar which is used as a tracking device for Jackie’s FMLA hours as well as non-FMLA hours. The calendar presented recorded Jackie’s absences from April 13, 2015 to current (FMLA hours were typed in, while non-FMLA hours were hand written in). A CNA visits Monday, Wednesday, Friday, and Saturday to assist her with showering, and on Tuesday, Thursday, and Sunday she has a neighbor or family member over as she engages in occupations in the bathroom. After the morning routine attends to her role as pet owner taking care of her cat’s needs. BK has a habit of misplacing her reading glasses and cell phone, but always has her Life Alert around her neck. She has weekly visits from her son and sister as they both help with cleaning, while also spending time with BK at the park or watching their favorite television shows. These visits from family are extremely meaningful to BK as she wants to fulfill the demands of her roles as mother and sibling. She visits the senior center to engage in social participation and leisure activities on Mondays and Wednesdays. In summation, Children’s appropriately notified BCBS of this inpatient admission on 01/08/2015. On 01/09/2015 Children’s submitted the clinical information to DeAngela Pearson BCBS UM Nurse to seek appropriate authorization. Children’s was not advised until 01/15/2015 that DeAngela was not handling this patient’s case due to it being an International account which was after the patient was discharged. I left a message for you on 7/14/17 regarding a possible linkage between Good Shepherd’s Prevention programs and St. Dominic’s Non-Medicaid Care Coordination program. The Care Coordination program works with children with Severe Emotional Behaviors who do not qualify for Health Homes. The idea for the linkage was to provide St. Dominic’s community support for children who are ready for discharge, but would need additional support. It has come to my attention that St. Dominic’s has a Prevention program, and it is the Department of Health’s expectation that they refer the children from the Care Coordination to their own Prevention program first.
