2.0 MATERIALS AND METHODS
This was a descriptive, cross-sectional hospital-based study, which was conducted at Mnazi Mmoja Hospital – Zanzibar in different adults wards to assess nurse`s knowledge, practice, as well as challenges faces when caring of critically ill patients. The quantitative approach was used to collect and analyze data from the study participants. The hospital is located in Stone Town, the historic center of Zanzibar City. It serves outpatient services/clinics, specialized clinics, as well as several wards for inpatient services. The hospital attends over 600 outpatients and 71 admissions on daily bases. The questionnaire included both close and open-ended questions and, comprised three sections in order to assess the following variables:- a) socio-demographic information, b) knowledge of nurses on the initial assessment of critically ill patient, and c) challenges face on care of critically ill patient. Observation check list was used to asses in getting more information practically which is not possible to capture by using questionnaire. In this regard, the researcher observed how nurses give care to patients directly during their nurses activities. The checklist consisted of four columns. The first column was for serial number of activities, second column was for skilled observed by researcher, third column for performance of nurses which contain two parts Yes and No and the last column for remarks. The content validity of tool was achieved by having the questionnaire reviewed by the professional nurses in the field of critical care and trauma and research experts to assess relevance of tool items and study objectives as well as face validity the researcher discussed together with professional nurse specialists if the questionnaires are clearly worded, well explained and if it addresses what it is meant to address. To ensure reliability of instrument, the questionnaire was pretested among nurses who were not included in the study, pretested was done to 17 nurses at Mnazi Mmoja Hospital (MMH) Zanzibar, it is a 10% of the sample size to check the clarity of questions and to give clues on how much time was spent to complete questionnaire. The data collected during the pretest was not included in the study.The data was collected by the researcher together with assistance during working time. During this time, the patient is unattended and while the nurse gets ready many issues can arrive and with critical patient time is valuable to their care. Another encounter issue is that report starts before going into the patient room. Staff were welcoming and, despite the organising of the observation taking a long time, the observation gave good insights in to patient experiences and leadership. I think the observation was useful to my partner and I, as it made us feel as if we now had a better insight in to patient experience and its effects on care. The experience itself was good; it was useful to gain that insight. One thing I did realise that was although we thought it went well, the insights we gained may not have been a true reflection of patient experience. To combat this, we could have asked more individuals to conduct the observations on various dates at various times.
Dr. Jean Watson’s theory of care addressed the nurse to patient ration, according to the method “nursing is positioned with caring of the sick, prevention of sickness, restoration of health and promotion of health. This process includes the process of assessment, plan, intervention, and evaluation. On the review, the nurse observes, identifies, review problem(s) and forms a care plan that will be used in appropriate nursing care. When the nurse to patient ratio is low, the nurse will not be able to perform this assessment. This will result in a reduction of patients’ outcomes, medical errors, frequent re-admissions, patient deaths.

Abstract: As the medical field continues to advance in technology, efficiency, evidenced based systemastic structure, inevitably the expectations and the responsibilities for the nurse only increase. In order to be competent withn their role, a nurse must stay focused, perform interventions quickly, safely, resourcal, and effective, the risk of desensitization in unconsciously lost due increase the timely and sufficient expectations. Nursing assessment has a significant role in providing effective, accurate and safe nursing care in clinical practice. Nursing assessment is the first stage of the Nursing Process. It is used to explore the physical, psychological, spiritual and social aspect of the patient’s life. It is therefore a holistic and systematic guide for nurses to obtain a greater understanding of their patient’s wants and needs. Based on the findings, most of the participants (64.5%) were aged 30 - 39 years old. 84% of the cases were female and 97.3% had a bachelor’s degree. The average perceived knowledge of nurses was 2.43 ± 1.01. The nurses had the highest familiarity with triage and lowest knowledge in epidemiology and decision-making.
