Hope you had a wonderful New Year. I want to thank you once again for giving the opportunity to interview at St. Joseph’s Family Medicine Residency Program on September 29, 2015. I enjoyed my interview at your program and meeting with the faculty and residents. Also, I appreciate you taking your time to give me a follow-up call last week. I was very impressed how close-knit the program was and how smoothly the faculty and residents interacted.
These experiences are just two of many that have influenced my decision to work in a similar environment upon my completion of medical school.
Serious Mental Illness (SMI) and Substance Use Disorders (SUD) each involve symptoms that can be rather debilitating. Approximately one half of the individuals diagnosed with a severe mental illness such as schizophrenia or bipolar disorder experience co-occurring substance use issues (Brunette et al., 2008). Individuals with a dual diagnosis of SMI and substance use are particularly vulnerable with complex service needs (Tsai et al., 2009). As a result of the high prevalence and serious consequences of these disorders, there is an increasing need for comprehensive treatment options to simultaneously address both issues.
I have completed an ample amount of time researching this career, shadowing providers in this specialty, and working along side these professionals has allowed me to determine without a doubt that this is the right career path for

I wanted to apologize for the comment that I made during the meeting. I failed to realize that I may have come off a bit defensive when you guys were trying to get feedback on why our schedule had recently fallen apart. Closing the day with 5 patients is a sound expectation therefore, I hope that my comment was not taken the wrong way. If it was , I sincerely apologize because
I was glad, however, that my instructor allowed us to pair up that day with our patients that way we could learn from each other. I enjoyed talking with my patient, and hearing about his military service, and his family.
Today was my seventh visit at the Woodhaven Care Center. Both my CI and I, were scheduled to leave at the same time today, so for a change we were extremely busy trying to get as much done in a half day as possible. In this short time, I worked with five patients. I had four of the five go through various therapeutic exercises, gait training in parallel bars, and up and down a flight of stairs. Since a few of these treatments were new areas to me, my CI performed the action once and I completed treatment with the patents.
It excites me to know that the knowledge I acquire at Cornell can one day be used to cure diseases and save lives. Alongside my academics, I enjoy conducting research and helping people through medicine. I plan on combining research with my love for medicine to make positive differences both on and off campus through innovative leadership and service to the people around me; I plan to use the education received at Cornell as well as the skills of networking and working with a team throughout my entire professional and personal life. This summer, I volunteered with an internal medicine physician, as well as third and fourth-year medical students at Doctor’s Community Hospital in Greenbelt, Maryland.
