What advantages and disadvantages of POMR, SOMR, and SOAP charting influence medical records and why?

POMR (problem oriented medical records) Advantages: Focuses on pts care and their problems. Relevant data related to a problem all in one place making it more easier and available for all who are providing the care. Evaluates staff performance of quality patient care.

Disadvantages: hard to Track trends inpatients data. A lot of flow sheets required.

SOMR(source oriented medical records)

Advantages: Organized for each department to locate.(ex. labs with labs ) Section for documentation and easy for adding loose papers.

Disadvantages: Can’t determine all patients problems and treatment fast enough, and must look in each area of the chart.
