Safety of the public, patients and staff is paramount and crucial to avoid more accidents. The following factors of threats can potentially harm us. Natural environment (soil, air, water) The regulation of fresh air and enough ventilation is needed in the hospital for the well-being of the staff, patients and public. Water supply might not be ideal for drinking or consumption, thus affecting the patients in the hospital as water need to be filtered and rationed in order to meet the demand of all patients. Extra water filtration devices have to be bought in this case. The growth of harmful bacteria in the soil will affect the air we breathe in. Especially when the public sit on benches outdoors, exposing themselves to the bacteria will directly affect their health. Soil bacteria like C. tetani spores are common in Singapore’s hot weather climate and it will cause tetanus (Wiki). Infectious wastes can come in different ways; pathogenic or pharmaceutical. Without proper sanitation and disposal, bacteria from the waste will spread to the patients or the staff handling them. Pathogenic waste that comes from humans and carcasses can spread harmful bacteria just by inhalation, accidental skin contact or even ingestion, thus it must be kept properly. 7 / D.P7: Explain how different procedures maintain health and safety in a selected health or social care setting Maintaining health and safety in health and social care is extremely important to ensure the health, safety and wellbeing of all their service users as well as other individuals service providers may come in contact with in the setting. There are several procedures that help to maintain this health and safety however they can all vary between settings for example, health and safety procedures will be slightly different and more focused on certain areas in hospitals and especially in paediatric ward compared to in drop-in centres where the needs and risk to service users are slightly different. Some of the procedures used in health and social care to maintain health and safety include; infection control and prevention, safe moving and handling of equipment and individuals, food preparation and storage, storage and administration of medication and storage and disposal of hazardous substances.
When dealing with substances, they will have a potential hazard which can affect the health care setting. The substance can be a potential hazard in the residential care home as there is clinical waste and cleaning products. As the nurse and the staff member will have to make sure that the cleaning products is locked in a room so that the visitors and the residents won 't be able to go in and touch the chemical products. There is a law called COSHH Regulation 2005, which stand for Control Of Substance Hazardous to Health regulation 2005. COSHH is when the hazardous substance have to be handled in a right way and it has to be stored in a minimise risk which is present.

The primary prevention is the best way to eliminate the potential for exposure. Since hand washing is the most effective mean of spread of infection, it would be my primary goal to increase the compliance of hand hygiene among healthcare workers, but also an extensive education of patients and family members on hand washing before and after touching the patient as well as afar any contact with any potentially contaminated materials (surface, body fluids or respiratory secretions). Mandatory education of patients, visitors and healthcare workers, across the system as well as cross department compliance practices are single best mean of preventing the spread of infection. For example, every patient and family member can be educated about hand hygiene, use of PPE-personal protective equipment (face mask, gowns and gloves). Although, the practices are already being utilized, I believe the compliance is poorly monitored.
The concern for safety has become a bigger and more important issue, and these two departments are forming a relationship. Although it has been the tradition for these two departments to work separately, they both have a common goal, to oversee the safety and excellence in healthcare organizations. Some smaller organizations have always had the same person control quality and risk and remained successful. These days, we are seeing a lot more collaborations, goal sharing, ad idea exchanging among these two groups (Perry, 2007). Risk management is critical to every organization.
Through the learning from week 1 and week 2, I have learnt that the most frequent adverse event in health-care delivery is health care-associated infections. It is essential for us to follow the infection control practices that both patients and us are at a risk of being infected. Standard Precautions involve the use of safe work practices and protective barriers, for example, the use of personal protective equipment(PPE). At first, I think Standard Precautions are very easy. Everyone knows PPE can protect us from infections and hand hygiene is important throughout the process.
A Medication error is defined as any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is under the control of the health care professional, patient, or consume. Therefore, any form of error that arrives within the healthcare system is deemed unacceptable. Now by understanding what a medication error entails, nurses are better able to place emphasis on how to prevent medication errors. It is important to prevent as many errors as possible when administering medications. Hospitals that accommodate high numbers of medication errors receive less funding and support by fellow agencies.
Safe medication administration is a big aspect of nursing care, because if medications aren’t given safely, then it can lead to some serious adverse effects to the patients. There are many things that can go wrong, and that’s why nurses have to be very careful when handling and giving medications. Nurses can make mistakes, and give the wrong med, give it to the wrong person, or even give too much or too little of the drug. Careful medication administration can lead to not making big mistakes that can lead to hurting others. “Medication Administration is a complex multistep process that encompasses prescribing, transcribing, dispensing, and administering drugs and monitoring patient response.”
This was based on quantitative safety results and Memorial Hermann prove to be that leader thus receiving the National Health System Patient Safety Leadership Award. This award proved that Memorial Hermann is a striver of excellence and values the safety of their patients. When hospital facilities have poor management, it can present itself in various forms, it can be reflected through patient care and the overall morale of the nurses. A hospital with will lackluster leadership may need to reevaluate and make necessary adjustments. A manager that is a focus on patient safety and the wellbeing of the nurses can contribute to increased productivity.
P6 Explain safe working practises necessary in preparing, cooking and serving food in the health and social care environment To be able to work in any food environment in the Health & Social Care setting you must first do a food hygiene course and they all come in different levels and categories, these are basic food hygiene to level 1 Food Safety, preparing drinks and snacks, for example, cups of tea, hot chocolate, making a sandwich or serving a piece of cake or fruit, Level 2 Food Hygiene, preparing and serving meals with hygiene training for those preparing and serving meals. The main reasons for developing a HCAI are poor hand hygiene by healthcare staff, medical device related infections such as intravenous lines and urinary catheters and the overuse or improper use antimicrobials. Hand hygiene is the cornerstone measure to prevent healthcare associated infection (HCAI) and it has been shown that MRSA is primarily transmitted from patient to patient via the hands of healthcare professionals (Donskey, 2009). Although hand hygiene is the simplest, most effective and most cost effective way to prevent the spread of HCAIs (Pratt et al. 2007) (Kilpatrick et al. 2013), adherence to hand hygiene among health care professionals remains low worldwide (WHO, 2009).
