Respite Stay Joe and Judy Baldwin have a two-week vacation planned in late June early July. Kathy will go to the Hensgen Home during this period of time for a respite stay. Joe will access the trust fund to cover the $130.00 per diem. During her time at the respite stay, Kathy will be expected to participate to her fullest abilities, staff will not cater to her and encourage her to participate in everything. It is the teams hope that she gain an understanding of the reality of residing at the Hengsen Home and will choose to remain living in the community.
Successful CC =1 CM received a call from Katlyn (Clinician at EMYL) informing CM that Tavian will have a new clinician at the end of October. Katlyn stated she will keep CM updated on the new clinician at a later date. CM was informed that Tavian is has been involved in gang affiliation and Katlyn does not agree with Tavian going home on passes. In preparation for our meeting tomorrow with Tyron Burnett and his mother Trina Burnett, I wanted to lay out a narrative about their experience with us which has led to this Patient/Family meeting at 10AM tomorrow. During their first visit to the ER on June 22, 2017 Tyron came to ER with his mother with complaints of groin pain. They were then told of his Chlamydia diagnosis, but he was in fact treated for both Chlamydia and Gonorrhea. They was not told of the second diagnosis. During this first encounter his mother made it a point to ensure the patient changed his contact number was changed from the house phone number to his mobile phone, so that the information would not be intercepted by anyone else in the family home, specifically his grandmother.
First, thing I did was have Ms. Pedone tell me exactly what they do in the morning. By doing this we established that Jordanna is home by herself for roughly 2 hours. Ms. Pedone clarified that during this time she stays in constant communication with Jordanna by phone. She calls to wakes her up, tells her to get ready and then calls back repeatedly to check in to make sure Jordanna left out for school on time.

Client must maintain employment and submit pay stubs weekly .Client is expected to comply with savings and have saved enough for one month 's rent and security by 12/18/16, Client is to seek for housing and obtain housing by 1/18/16.Client was reminded she must meet with her Housing Specialist Mr. Brown on a bi-weekly basis, comply with all housing requirements and explore all available housing options. Social Service: Client must meet bi-weekly with Case Manager for ILP Review appointments.

Early in her interview, I remember her explaining that her owners were not only kind to her through their teaching of the alphabet, but also to her parents, specifically her father who was continuously supplied with tasks from their master for additional money after gaining freedom. This struck me as particularly generous, because not only did the master keep in touch with the family after the abolishment of slavery, but he tried to provide the family with the most meaningful opportunities in the “real world.” Bernice told me that her mother remained on the plantation for an additional year after freedom, and later when her mother became ill, their old mistress would travel over six miles with gifts every day until her death. Bernice’s explanation of the close bond between her family and her old masters made me realize that it was not only the slaves who felt connected to their owners long after freedom, but some plantation owners grew to care their slaves like family of their own. So although they supported the horrible institution of slavery itself, it did not prevent relationships between their slaves and them.
