I- CM continued to assess client’s mental health. CM inquired if client attended his appointment (3/30) with Laurie Garza from LAMP VASH. CM conducted a 30 day ISP with client. CM continued to provide emotion support to the client. CM completed an overnight pass request for client
R- Client presented well-groomed and with good hygiene. The client appeared calm and made good eye contact, was cooperative, and open for discussion. His affect was appropriate and his mood appeared euthymic.
Housing: Client has been assigned to a LAMP VASH worker, Laurie Garza. Client missed his first assessment appointment (3/23) with the LAMP VASH worker, because CM provided him the wrong address. Client was able to reschedule his appointment with Laurie Garza at an earlier date (3/30). Client stated that his appointment with Laurie Garza went really well, but he has to provide her with a copy of his EBT verification letter. Client stated that he has to visit the DPSS office in Orange County to obtain his EBT verification letter, and provide Laurie Garza a copy of it at his next appointment (4/7).

CCIB received a SOC 341 from APS for the following residents: Daniel Hourihan (DOB: 10/03/54), Gerardo Guerrero (DOB: 01/02/95) and Gerald Gaither (DOB: 06/14/59). Per the reporting party, Lee Nln, the provider/Owner Cheryl Oliver has been advertising her independent living facility as a board and care. RP stated that her clients have been paying the board and care rate and the client require medication management. RP stated that in April 2018, client Daniel Hourihan moved to another facility. The rent for Daniel was sent to Ms. Oliver who continued to cash the rent checks for Daniel.
CCIB Intake received the SOC 341 for resident Clothide McCord. Reporting party Tonya Hill Conservator- Deputy Public Guardian 222 W. Brookside Ave., Redlands CA 92373 (909) 798-8500. The reporting party and Public Health Nurse, Sandra Garcia visited Clothide at the facility on 12/13/16. The resident was found barely responsive and dehydrated.

Successful CC =1 CM received a call from Katlyn (Clinician at EMYL) informing CM that Tavian will have a new clinician at the end of October. Katlyn stated she will keep CM updated on the new clinician at a later date. The referrals were not received or processed, however you completed a Case Management Visit on 4/1/16 regarding information contained on the referrals. How would you like me to handle the referrals. I can attach a LIC812 on your LIC809 if you prefer. I don’t believe I need to process a complaint due to your case management visit.
She said she thinks she was on drugs at the time. Ms. Morgan has a valid case with the agency dated 7/18/2015 for Dependency. Ms. Morgan is diagnosed with Bipolar and Schizophrenia. Ms. Morgan is not taking medication at this time and she is not receiving mental health care. Ms. Morgan is supposed to receive care through Central City Mental Health Clinic, but missed her appointment and never rescheduled.

Therapist met with the client for individual therapy at school. Therapist did a check in regards to symptoms, mood, thoughts, emotions, coping skills, the goals that he achieved, and behaviors since the last session. Therapist processed the client's negative thoughts. Therapist used open-ended questions to address any concerns the client may have. Therapist encouraged the client to keep motivated during the stressful time, especially when he has negative thoughts, which stats usually with negative thoughts, argue with his sisters, or with his aunt, or if he has been triggered by any internal or external thoughts.
On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment. On 11/27/16 CM went to conduct the Safe Sleep inspection with the DHS Compliance Analyst Jeanette Chirico, while in the second floor we went to unit #222 assigned to Ms. Tiana Footman and her three children.

On 11/15/15 at approximately 1257 hours, I, Offcier Artaz responded to 727 Labor Street in reference to a disturbance. This location is in the City of Delta, County of Delta and the State of Colorado. I arrived on scene , and contacted Deborah Dugas (DOB 08/16/71). Deborah told me that her and her son, Spencer Bachman (DOB 11/20/88), had gotten into a verbal agument. Deborah advised that Spencer has a restraining order, that prohibits him from drinking alcohol.
