When a patient is admitted to my unit, I am working with the clinical care team including physicians, nurses, physiotherapist, occupational therapist, dietitian, and hospital based care coordinator to ensure that our patients have the resources to help them thrive when they return home to their own community. We are able to achieve this by constantly communicating with one another as well as communicating with our patients and their families to determine which community resources would best suit each specific patient. We have referred our patients to various community programs including the Home Care program, the Geriatric Program Assessment Teams (GPAT) and the Geriatric Mental Health Teams (GMHT) program, and Community Therapy Services. All of these programs are designed to assess the patients in their own homes to determine how the patients are managing at home and determine whether the patients would benefit from additional services and resources. At Grace Hospital, we work to respect the wishes of our patients through self-determination and patient-driven care through the involvement of patients and their families in the patient’s plan of care. As a social worker at Grace Hospital, I work from a strength-based, anti-oppressive framework. It is important for me to understand each patient’s ecosystem (i.e. support system), their own personal values and beliefs, and the knowledge and skills that they possess. By working from this framework, I am trying to develop a mutual trust and respect for one another which enables me to work together with the patient to develop an appropriate plan of action to help resolve their identified issues and concerns. When the clinical care team is working with a patient, we strive to provide them with all of the information and options to allow them to make an informed decision. In the staffs eyes most vision, mission, and core values listed by agencies tend to overlap or state the same idea and are tailored to the specific intuition that claims the views are individualized. Thank you for reaching out to me and explaining the current situation at Grace Fellowship. Even though the church may be experiencing some difficult times in terms of finances and membership, I know that God is “able to do far more abundantly than all that we ask or think, according to the power at work within us” (Eph. 3:20, ESV). Grace Fellowship is a part of His Church, and I believe God has a plan and purpose for the church, yet to be fulfilled within the community of Denton. Mission statement is an important aspect of strategic planning however concern has been raised regarding its impact on firm performance (e.g. Krohe 1995; Mosner,1995). Authors (Bart and Hupfer, 2004) recognize the need for empirical investigation of mission statements that is specific and provide practical outcome for the locals. (e.g. Gergen and Thatchenkery, 1996). People often fail to recognize that: […] business recipes…reflect the circumstances in which they developed and cannot simply be transferred to qualitatively different environment (Whitley, 1992, pp.125-6).
When it comes to decision making the patient has the right to exercise autonomy. The patient has a right to know their treatment plan, alternative treatments, choose to implement the treatment of choice or chose not to. The patient also has the right to stop the treatment plan that was implemented, but he also has the right to know the consequences of his decisions when he chooses to refuse, stop or postpone treatment until further notice. As I mentioned previously the patient has the right to exercise autonomy, but they are also responsible for the outcome of their choices. If the patient chooses to leave AMA or be transferred to another facility they also hold the right to do that, but we as nurses must always tell them the truth regarding their decision as well as the outcomes of those decisions.
I've realised that assisting the vulnerable is something I’m passionate about. , a skill I'm very keen on developing. In the role as a support worker, I gained confidence and experience on how to effectively support people with physical and mental challenges in a person-centred manner, ensuring that they maintain their identity and dignity without bias and prejudices. I write reports on service users' well-being on a daily basis, monitor their independence goals and provide any necessary assistance, and help them in developing greater independence while also improving their employment abilities. I've had the training to learn how to effectively support service users with health and well-being issues, as well as care review meetings to learn how to assist service users in evaluating their circumstances and outlining long-term objectives and intended outcomes while encouraging empowerment and self-confidence.
Patients have a right to complain about the doctor's refusal to the Management. Provision of Treatment requires patient’s choice and informed consent. Even if a patient has signed a general consent clause, the patient can still refuse medical treatment or procedures. However, in exceptional or emergency situations a doctor may be legally justified in performing surgery or providing treatment without the patient's consent. The patient should be competent and capable of making such a decision to give a consent.
