Guided Reflection #1 The overall experience of this patient experience has been quite memorable so far. In terms of contributions contributing to our patient’s quality of life, our contributions have been more practical in nature. Our patient reported having urinary issues and wetting the bed after taking the diuretic. We suggested he time his doses so he will know roughly around what time he will have to go bathroom. This way he will have enough time to make it to the bathroom and avoid urinating elsewhere. Another positive was explaining the indication for Gabapentin. The patient was not taking Gabapentin and was unsure what it was used for. After informing him of the name and indication, I believe we have informed the patient about his
Critical Thinking: Tanner (2006), had introduce the term ‘thinking like a nurse’. When I read this article I was wondering what it means and takes to think like a nurse, I am a nurse by profession and yet I don’t even know what it means. In order to be a professional nurse, nurses are required to learn to think like a nurse. To my surprise, to be able think like a nurse, a nurse got to clearly defined and understands what is critical thinking and clinical reasoning. Both terms are powerful terms and these terms explain the mental processes nurses use to make certain that they are doing their most excellent thinking and decision making for their patient’s better outcomes.
Reflecting on the past seven weeks I have acquired countless knowledge, which I will use to further strengthen my profession as an Advanced Practice Nurse. The course allowed me to think beyond my current practice knowledge and acquired innovative ways to evaluate the situation at hand. The learning objective in program outcome four helps set standards that I will use to guide my clinical practice to meet various healthcare needs. Using the case studies has helped to further enhance my knowledge on disease physiological state, using differential diagnosis, disease manifestations, and clinical presentation. It has also taught me the skills on how to differentiate between similar diagnoses to properly identify the problem and treat the patients.

This reflective writing will discover and highlight the major patient care concept in relation to my personal experience and feelings. Throughout this essay, my reflection will be based on how well I was able to treat the patient who was suffering from severe pain in surgical ward. I will further explore the pain assessment as conducted with one 70 year old patient whom I will refer as Mr X. while describing my overall experience with a single patient, I will also reveal and share some questions and ideas which has provoked me to know more about pain assessment criteria in general. Discussion I will be using Gibbs reflective cycle as a guide for my essay. Gibbs reflective cycle is one of the most used and popular model of reflection which includes the steps and detail including the description that reveals the facts and situation of the case that might take place when incident occurs (Gibbs, 1988).
The first section is about the introduction of my speech. The specific purpose and central idea in my speech were clear. I would say I gained most of my audience’s attention in the introduction and that it was effective. My attention getter does orient the audience to my topic. I think it snagged most people’s attention and did motivate my audience to listen.
This week I met with a social worker at Cherry Health which is located a few blocks from Heart of the City. When I met with the social worker we did a role playing exercise. She played a client that was experiencing depression and sadness. In addition, she role played as a client that she typically work with at the agency and I role played as the therapist. I enjoyed the role play because I was able to receive feedback about my progress.

I cannot express enough of the positive feedback I receive from patients, families, and floating staff during my leadership rounds. You are truly an asset in the unit in making the patient experience rewarding during the hospital stay.
I am a nurse in one of the aged care, one day I started to give shower to my elderly client with love and care but she refused to shower. After that I spend some time with her. I talk with her politely, give her some time and try to figure out her feelings and reason behind refusing shower. She was not comfortable to shower before breakfast but likes to be shower after. So I respect her feeling thus gave her shower after breakfast as she prefer.
Client-centered therapy is unique in that it makes me feel connected to my clients on a person-to-person basis. This form of therapy places me in a conflict between my values and emotions. As a therapist, I would need to separate my client 's behavior from their persona, and I would need to address them as an individual not defined by their actions. From my perspective, it would be difficult to display unconditional positive regard to a client that has committed many harmful and violent crimes. Introduction/Description The purpose of this essay is to deliver a reflective account of the learning and educating actions required to change a stoma bag. A third year student carried out the actions required to complete the task within a ward setting. The Royal College of Nursing (RCN), 2015 describe reflection as a process where nurses are given the opportunity to reflect on significant events that have taken place and explore their knowledge and practical skills in an objective way. Using analysis and evaluating the nurses actions, provides a learning opportunity, allowing future practice to be improved including professional development and patient safety (Nursing and Midwifery Council, (NMC) 2015).
Evolution from Novice to a Beginning Professional Nurse Looking back when I started my nursing school, I can proudly say that I have evolved both physically and psychologically. I was not sure if nursing was the right choice for me at first because I had difficulties interacting with people outside my family circle. The first semester was challenging for me because I did not know how to interact with my class mates and felt isolated each time there is a class activity. I joined a study group later that semester and it has been an excitement ever since.
Over the past one and half month, the class of PDE 502 (Counselling and Career Education) has taught me some major lessons for life in dealing with the clients in response to their emotional needs. The role of a counsellor is not unlike that of a friend where by it is nurtured by being in each other’s company, talking over everyday issues and sharing feelings. However, what sets a counsellor apart is their experience and the ability to apply counselling theories and techniques to assist people in gaining awareness, insight and explore ways of solving their own issues.
