Summary Human error is a concern alarming how people execute duties on and off work. Some factors are the limitations, capabilities, and skills of human being. In aviation, errors exist from the ground like mechanics or air traffic control to the air like the pilots or crew coordination. When assessing an accident that occurred it can be caused by equipment failure as well as a mechanical problem or just simply the person made a mistake.
Problem Statement
On November 17, 2013 a tragic ending happened on Kazan (Russia) airport. A Tatarstan Airlines Boeing 737-500 was flying 44 passengers which included the President of Tatarsan’s son and 6 crew members from Moscow Domodedovo to Kazan (Russia). There were no issues on the flight until the

To work as a pilot on an airline company you need to have a few requirements such as: some type of aviation degree, up to 2 months of ground training and more than 1500 hours of flight experience. The license and certification requirements; are commercial pilot’s license, instrumental training as well as airline transport pilot certification. Having the requirements or certification is just the beginning. As a pilot you also need to have important skills such as: strong communication, problem solving, good reaction time or overcome the issue, and able to operate aircraft computer plus the navigation system. Being a pilot is one of the most challenging jobs, due to being responsible for the cargo or personnel but also having possible hazard during flight like weather problems or being fatigue. Once be looked at for a position, the pilot does an extra 6 to 8 weeks of ground training with about 25 hours of flight training to give the individual more experience before being hired by the airline. All this training and certifications are required not just to hire pilots but also to keep everyone safe on the air when traveling. The advantage of this training is to keep the pilot well trained for different types of environments also being trained for the (what if) scenarios and overcome them with no issues. Being trained on the right equipment and aircraft is the pilots’ job to be able to understand what to do in the air. This will make you more aware when a hazard can occur and how to manage the obstacle. One of the disadvantages of Tatarstan Airlines was that the pilot was not properly trained on the Boeing 737-500 aircraft. Another concern was that the crew was not mentally trained for missed approach procedures as well as not paying attention to detail like the autopilot being disconnected. There were a lot of violations coming from Tatarstan Airlines and the lack of
Through the use of recorded patterns Gladwell makes the belief of, more or less, seven errors on a plane will result in a crash. Gladwell embraces repetition to present the problem of pilots not communicating consequently making errors of flying a plane “and then another and another and another and another” using this rhetorical structure he presents the number of small errors that have happened that lead to a crash (Gladwell,184). He emphasizes the extent of problems on the plane to create an image of chaos on the plane. The use of anaphora created a feeling of anxiousness as the problem could’ve been prevented “one pilot knows something …one pilot does something wrong” with all the problems that have occurred and the pilot and co-pilot not
With security concerns paramount, each plane had to be searched and every passenger
The Soviets were furious that the U.S. was doing secret flights over their military basis even though just a few years earlier one of their own spies was caught in the
On September 25th 1978 a Boeing 727 just minutes before landing crashes in San Diageo, making it the first site of the biggest aviation disaster in the US history. Pacific Southwest Airline Flight 182 was having an early morning flight on the coast of California. It was travelling from Sacramento to San Diageo. Co-pilot Robert Fox, a 9 year old veteran was in charge of the plane alongside Captain James McFeron who was with PSA for 17 years, he was appreciated highly by his colleagues for his flying skills. It was the second flight of the day for both men.
These classes take up to 2-6 months. The training usually includes undergoing urgent situations in order to prepare you for any type of those situations that may occur in the career of an
All the experiment was depending on fortune and luck and can not be successful at all because the performers do not have proper direction to perform the experiment. For the experiment, workers should have proper equipment, standardise and should have power to do work their own way. It is also concluded that the quality of production might be great in future by use the present statics and create a quality control chart and solve the problem in specific problems which effects the project. The common wisdom is that if only people did not make so many mistakes, there would not be so many problems. But even with the variation contributed by the people reduced to zero, there are still too many red
In the article The Worst Mistake in the History of the Human Race the author Jared Diamond explains how the development of agriculture in humanity affected the evolution of our modern society for the worst. He proved this thesis with sufficient points, however, the scientific evidence behind the Luddites’ beliefs are limited. The corroboration behind agricultural advancements being a substantial step for mankind is far more concrete than the opposing side. Livestock and cultivating vegetation was the most significant switch humanity has taken, and molded our world to what it is today. Paleopathologists have recorded that the health statuses of people became critically worse after the Neolithic Period, when civilizations switched to
8 Baby Girl is Lone Survivor in Mass Shooting Nytimes.com On the 15th April 1984 a man named Christopher Thomas went on a jealous rampage and murdered ten people in Brooklyn, New York. Seven of his victims were children. This mass murder became known as The Palm Sunday Massacre. He shot all the victims at close range with one of the two handguns he carried with him.
American Airlines Flight 1420 June 1, 1999, a McDonnel Douglas DC-9-82, dispatched as American Airlines 1420, crashed after it had overran runway 4R, at Little Rock National Airport. The flight claimed the lives of 11 people, including the captain, and 105 passengers received serious or minor injuries, including the first officer and flight attendants (NTSB, 1999). According to the NTSB, this accident was due to pilot error (NTSB, 1999). This report will exam all human interaction as well as performance, utilizing Dr. Scott Shappell’s and Dr. Doug Wiegmann’s HFACS model, so one may find the route cause of the errors, and prevent similar accidents in the future.
HFACs is used to identify the probable causes of accidents by laying a valid framework to investigators analyzing aviation accidents. The two human factor errors that contributed to the accident were fatigue and situational stress. The organizational failures may not stand out to untrained bystanders. These organizational failures must be identified because, even though they may not be easily recognized, the failures of an organization’s oversite can lead to other human errors. When human factors and organizational failures happen at the same time, these types of events are likely to occur.
American Airlines Flight 587 Title: Aircraft Accident report: In-flight separation of vertical stabilizer of American Airlines flight 587, an Airbus Industrie A300-605R aircraft on November 12, 2011. Author of this paper are Nitin Jayant (2012CE10371) & Jitesh Kumar (2012CE10355). Abstract:
Besides the issue mentioned above, the team from the airline were not able to build a relationship with outsourced suppliers. Finding a solution to the issues in pursuing multiple vendor sourcing strategies, the airline centralized IT department. This did not end there because the procedure of collecting information on IT needs and passing to IT for implementation was done in the same way for both local and spanned different branches IT requirements. This has caused the IT to work slowly in implementation and they were unable to respond according to the real business needs that were collected by the business
For worldwide airline industry, opportunities can emerge from new client expectations, items, business sector structures or regulatory
Gladwell gathered several sources of evidence to explain this phenomenon. The first part of the problem was communication. Transcripts were gathered from several planes in emergency situations and showed that the lack of proper communication between the captain and the first officer contributed to the accident. By giving a crew command and
9. Environments Like any other Industry, the airline industry is also affected by changes in its external environment. King III (2009) highlights that leaders are not supposed to compromise the natural environment and the livelihood of future generations. Environmental Factors can also have a significant role to play in an airline industry; like in the case of Prof. McPherson we observe the bad weather reducing his time by 1 hour and thirty minutes. In light of the environmental factors that affect the airline industry this Study will focus on the traditional Political, Economic, Social, Technological, Environmental, and Legal Analysis, often referred to as the PESTEL Analysis.
