7. HEALTH INFORMATION
Health information is the data on medical history, diagnoses, procedures, and outcomes regarding a particular person. The health information records contain patient’s history, clinical information, x-rays, lab results and notes. Patient’s health information can be viewed individually, to see if the health is improving or deteriorating, or as a part of a large data set to determine the health of the population if improving or deteriorating over the years. The medical intervention on how change the health outcomes could also be developed from health information gathered.

9. HEALTH EDUCATION
Health education is a process by which an individual or group of people learn to behave in a manner that promotes, maintain or restore health. It is a profession that educates people about health. The area within this profession are social health, environmental health, intellectual health, spiritual health and physical health.
It was defined by joint committee on health education and promotion terminology as combination of planned learning experiences based on sound theories that provide individuals, groups, and communities the opportunity to acquire information and the skills needed to make quality health decisions.
WHO defines health education as comprising of consciously constructed opportunities for learning involving some form of communication designed to improve health literacy, including improving knowledge, and developing life skills which are conducive to individual and community

PURPOSES OF PUBLIC HEALTH
• Assessment: this is done through monitoring and evaluation of the health status of the community and identify the health problems, in order to diagnose and investigate these health problems and health hazards in the community.
• Policy development: this is to develop policies and plans that support individual and community health efforts and to enlighten, educate and empower people about health issues and to solve the health problems in partnerships with community.
• Assurance: this is by enforcing laws and regulations that protect and ensure public health and safety. Link the people to necessary personal health services and ensure competent public and personal health care workforce when not available. Constantly researching for new insights and innovative solutions to health problems.
PUBLIC HEALTH NURSING
Public health nursing is a specialty practice of nursing and public health. Its focus is on improving overall health of the people within the community with emphasis on prevention, and attending to multiple determinants of health. This nursing practice involves advocacy, planning and policy development that will address issues of social justice. The nurse looks at the areas of concern in the community, assesses it, and plans ways by the concerns can be curtailed or resolved. The health concerns could be on infection control, health coaching, health maintenance, as well as home care visits for welfare
Assignment – There are five common purposes for medical records. List each of these purposes and provide an example of each in healthcare. Having good medical records is very important, for the proper care of patients. “Medical records can be used to manage healthcare, track healthcare, provide clinical data, meet regulatory requirements, and document healthcare” (Allen, 2013, P. 57). Without the proper documentation there is no proof that it was ever done.
#1- Compare and contrast the clinical uses of a health record with the secondary purposes of a health record. The use of Health Records are used by both, clinicians and non-clinicians (secondary purposes). Reasons to why clinicians may use a patient records are for confidential data such as patient care (diagnosis and treatment), chronological documentation of clinical care, method of cross discipline education, research activities, public health monitoring and for quality improvement activities. In contrast, non-clinicians may use is for non-confidential informational data such as billing and reimbursement, verifying disabilities, and legal documentation of care.

The recent issues regarding the health of the public is getting complex. There need to be new and effective ways to promote and protect health and prevention of the disease. Roles of the Professional Nurse The role of the professional nurse in today's healthcare setting continues to expand as our healthcare delivery system in the United States changes. Historically, nurses have delivered care in a system that has been "illness" centered. Increases in knowledge of disease, coupled with a desire to decrease the costs of delivering health care have shifted the U.S. model of care to one of prevention (IOM, 2010, p. 1-1). Data use agreements and connections between HIE organizations are also a concern with health information exchange. “The types of healthcare partners that are needed is a challenge when determining the trading area needed for a viable HIE” (PHII, 2005). Although there are challenges, there are opportunities with implementing a health information exchange system. One opportunity is HIE can improve reporting to public health and in return improve the health of the community.
This entry will discuss the DNP student peer-reviewed presentation at the Haitian American Nurses Association (H.A.N.A) monthly meeting on March 18, 2017. The DNP student had the opportunity to speak to members who were non-health professionals, nursing students, nurses, ARNP’s and DNP’s. The DNP student had the chance to prepare for this presentation with her mentor Dr. Marie Etienne. Preparation was done via a conference call. H.AN.A’s meeting was from 7:00PM to 10:00 PM.

With the development of learned abilities nurses are able to heal by easing suffering, treatment, and encouragement of the care of people, families, communities, and populations. Professional nursing roles entail practices based on evidence that are defensive, recuperative, and promotive when focusing on a holistic manner. Developing professional roles of nurses are highly recognized and nurtured. A nurse plays an essential role in the healthcare system.

The importance of epidemiology is that Epidemiologists identify and measure the health events within the population. This scientific field develops a variety of models of how an event may occur, in order to recognize and understand those events on a higher level.
Health Nurses need to emphasize preventative health, cure for diseases, and health promotion. Nursing Virginia Henderson defined nursing as: “The unique function of the nurse is to assist the individual, sick or well, in the performance of those activities contributing to health or its recovery (or to peaceful death) that he would perform unaided if he had the necessary strength, will or knowledge.” (Vera,2014) Henderson states that “The nurse is the only twenty four hour worker. We are the only people that will say we stay with you.
Importance of nurses A nurse is a health care professional who is engaged in the practice of nursing. Nurses are men and women who are responsible (along with other health care professionals) for the treatment, safety and recovery of acutely or chronically ill or injured people, health maintenance of the healthy, and treatment of life-threatening emergencies in a wide range of health care settings. Nurses may also be involved in medical and nursing research and perform a wide range of non-clinical functions necessary to the delivery of health care. Nurses develop a plan of care, sometimes working collaboratively with physicians, therapists, the patient, the patient 's family and other team members.
