O: I want to maintain independent living in place of my own. A: MHS and the member discussed the member’s current concerns. MHS recommend parenting classes for to the member. MHS discussed the member’s house cleaning with the member. R: The member was in a good mood today. The member began the session informing MHS that her one year old son had consumed a cigarette while she was sleep; and the member found the situation to funny. MHS expressed her concerns and advised the member that this is something that you do not take lightly and should use preventive measures to assure it does not occur again. MHS then begins to notice another safety hazard within the household such as cleaning products not properly put away and apparently within reach
On 10/22/2015 SO EMT Perez was dispatched to FC 418 regaurding a fallen female resident. SO EMT Perez knocked and announce his presence at the door and was greeted by the spouse of the resident who invited him in. The fallen resident in question was on the kitchen floor sitting upright, a Mrs. Barbara Murtishaw. Mrs. Barbara Murtishaw had fallen in her kitchen and needed a lift assist because she could not get up on her own strength. Mrs. Barbara Murtishaw stated that she was not in any pain and only wated to be lifted upright.
CCIB Intake received a SOC 341 regarding an incident which occurred on 5/12/16 involving client Christian (last name unknown) and client J.W. According to the reporting party (RP) Program Manager Jim Kaing at 3:10PM while Christian was in the kitchen eating a snack he suddenly rose from the kitchen table and walked into the "Art Room" and pule the hair of client J.W. Summary Recently a group of therapist administered the SAFER v. 3 to assess MM’s safety in her two-story home in Blue Bell, PA. After completing the assessment it was determined that MM has several areas that were deemed either “mild” or “moderate” problem areas. Much of this has to do with her intellectual disabilities or her mild CP, which causes her to toe walk. Of note, however, is the face that MM lives with her two parents and has an aide that comes to her home four times a week.

On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment. On 11/27/16 CM went to conduct the Safe Sleep inspection with the DHS Compliance Analyst Jeanette Chirico, while in the second floor we went to unit #222 assigned to Ms. Tiana Footman and her three children.
