Medication reconciliation assignment was an individual activity that I had to perform as a part of a course requirement. For this activity, we had clinical simulation lab organized with standardized patient. In simulation lab, I had to refer patient’s chart that includes his home medications and then interview standardized patient and get all detailed information regarding his medication schedule including name of medication, strength, dosage form, route, frequency and any adverse event associated with any medication patient is taking. After interviewing patient, I had to update patient’s medication list in to the patient’s chart and based on my clinical knowledge if I found any discrepancy in the patient medication list then I have to come

Continuous advancement in technology is also changing the healthcare system to ensure patient safety and provide high quality patient centered care. The hospitals are adding more and more computer assisted devices and the nurses are facing new challenges every day. Change in the workplace means making changes in the work environment that is different from the current state. Implementing a change can create anxiety or fear of failure in nurses, which may lead to a resistance to change practice. “Changes to a system may be anticipated and planned, or they may be sudden and unexpected” (Yoder-Wise, 2015). Which then directs me to my clinical question, does accurate medication reconciliation (intervention) influence patient safety (outcome) in patent’s who have adverse drug events (problem) over a one year within ambulatory clinic settings (time)? My PICOT supportive research question has been further evaluated from the journal article, “Ambulatory Medication Reconciliation: Using a Collaborative Approach to Process Improvement at an Academic Medical Center” written by Keogh et al. (2016).

When I was in first grade at the age of six years old, I tend to get sick a lot of times. So when I did get sick my aunt will go buy me some OTC children’s ibuprofen for my fever’s, then I will read the directions on how to take it, and read the package insert that involves the warning on what not to take with the medication, and side effects of the medication.I always wanted to become a Pharmacy Tech when I was little to fulfill my dreams. Pharmacy Tech is the reason why human beings stay healthy by dispensing the medication and making sure medication on time.
