On September 6, 2015 at approximately 8:21pm I, Deputy Tucker George, checked en route to the Shell Gas Station in Hawkins, Texas 75765, Located at 102 East front street in reference to an assault call. Upon arrival I located the complainant Michele Turner inside the Shell station sitting at a small table on the North wall of the building just inside the door. Michele Turner appeared to be intoxicated upon my arrival and displayed multiple signs of intoxication throughout the process of conducting my investigation. Michele Turner was unable to provide detailed information in regards to her assault, but did state the house at which she had been and was assaulted was a known dope house. On 11/07/2017, the agent received a Law Enforcement Contact form indicating offender Joe A. Slugger was at the K&D Tap on 11/06/2017 having unauthorized contact with Sandra Williams. It was stated in the LEC form that Joe Slugger was drinking alcohol and will be referred for the charge of felony Substantial Battery by Badger Police Department after he was witnessed to hit Sandra Williams with his open right hand to Sandra William’s left side of her face and then grab Sandra Williams by the back of her head and slamming her head onto the bar counter where she was seated. Sandra Williams was observed having a visible red mark on her face and a laceration on her forehead that later required eight stitches. On 11/08/2017, the agent received a police report from Badger Police Department Officer Konkol (Case number 1-98-00456723) stating Officer Konkol and Officer Sandner were dispatched to the K&D Tap (223 W. main Street, Badger WI) where they made contact with Joe Slugger.

Taser International, Inc. (defendant) Facts: Darryl Turner, a seventeen-year-old grocery store bagger, is the deceased in this wrongful death case. Turner was fired and refused to leave the store’s premises peacefully, the police were then called. Police officers used a Taser twice on Darryl while taking him into custody, after collapsing he was unable to be revived. Tammy Lou Fontenot was charged with administering Turner’s estate.
While on station after transporting Deadrick Allen, this officer (Ofc. J. Jaques #64) was told by Det Sgt. Sibley #42 to place Allen into cell #3. While in the process of searching Allen 's person before placing him into the cell I located a small ziplock style baggy filled with a green and brown plantlike substance in his front right small front pocket. After removing the baggy Allen began being uncooperative.
ON 05/01/2017 at approximately 1000 hours, Deputy Thacker and I, received a call from dispatch that the person living a t 7400 quail court, Tarrant County Tx, was tapping into the water line of their neighbors house at 7412 quail court, Tarrant County Tx. At approximately 1020 hours, Myself and Deputy Thacker arrived at 7412 quail court, I made contact with the owner Mrs. Rachel Gilliam. Mrs. Gilliam explained to me that she noticed an external hose connected to her water meter that was leading to 7400 quail court. I investigated the water meter belonging to 7412 quail court and saw that their was a hose connected into the water line that belong to 7412 quail court. I made contact with the owner of 7400 quail court, Brown, James (W/M, DOB 01/29/1989).
• The defendant (Mr. Brown) was not under the influence of a controlled substance but I (Officer Reed) noticed strange behavior of the defendant (Mr. Brown). • Upon opening the door to his home the defendant (Mr. Brown) fled from the front door in which Officer Reed and Officer Malloy pursued the defendant (Mr. Brown). • The controlled substance was located inside of baggies on the kitchen table. • The weight of the controlled substance was 1360.8 grams.
INTRODUCTION: This case involves the arrest of the suspect for possession of an open container of alcohol in public, in violation of PMC 9.24.030. The suspect was cited out. INVESTIGATION: On 01-12-16 at 0836 hours, I was working uniformed patrol with Officer Cass #2067.
While on patrol, I was checking Pleasant Hill Primitive Baptist Church on Needmore Road and observed a burgundy Tahoe sitting stationary, close to the woodline of the church. I exited my patrol car and approached the driver side of the Tahoe. I quickly recognized the driver as William Anthony Thomas, who has a felony warrant with our agency. Thomas was then detained until further investigation. Thomas name was ran through dispatch and they advised that he had a felony warrant for DUI-Felony.
At about 12:45pm, I observed Tina was screaming at Alyssa Wolfe and two other service crew workers “cherry-picking” on the R17. After my one-on-one discussions with all the service crew workers, it became apparent that Tina Williams was dissatisfied with the way the service crew workers were working as well as the fact that Christian Segers and Alyssa Wolfe switched alleys mid-shift (a switch that they claimed was approved by Ana Velez). They also said that Tina Williams was cursing at them while complaining about the way they were working. Tina Williams denies swearing at anyone and says she was just concerned about the alley. On October 09th, 2015 at approximately 5:30 PM, I, Investigator James Poffel was the on-call investigator and was contacted by dispatch in reference to responding to The Days Inn for a call of a kidnapping that patrol officers were out on. Officer Wilkins was the officer originally dispatched to t hat location and was on-scene awaiting my arrival. I spoke with Officer Wilkins while I was en route to his location. Officer Wilkins informed me they were sent to The Days Inn, room #104, at 900 S 32nd St to check the welfare of a subject by the name of Kayla Blevins. Dispatch received the call from Kayla’s grandmother, advising Kayla was being held at that location against her will, for a $1,000.00 ransom.
On 04/23/2016 at approximately 23:15 hours deputies discovered a vehicle parked on the bridge that goes over the Ninnescha river, in the 800 block of E. 110th AVE North Sumner County, KS. the vehicle matched the description of a vehicle that was seen leaving the area of a verbal disturbance with shots fired. Deputy Coon went and made contact with the driver as I gave the vehicle information to dispatch. The vehicle was a white Chevrolet truck with Oklahoma tag of 541KQY VIN of 1GCGC29R5VE127727. After the vehicle information was given to dispatch I walked up to the passenger side door and began to look inside.
On 05/06/17, at 11:43am, I Deputy Warden N. Christian was dispatched to 523 S Wayne Ave on a dog on dog or cat attack, no known owner. I arrived at the location and was met by Columbus Division of Police (CPD) and complainant Raquel Fuentes. I asked what happened; Ms. Fuentes stated that she was taken part in a community event by assisting with neighborhood clean-up with her dog (small white terrier) when a brown/white Pit Bull came out of nowhere and proceeded to attack her dog. With the assistance of passerby they were able to get the brown/white Pit Bull away from Ms. Fuentes dog.
This information that I am sharing with you is not to put on your shoulders another heavy burden. Is just to see if you know any church or person that can assess the following situation. This lady from Orange, Mrs Pat Turner, has a big Oak tree partially falling down in her property due to was hit by a lighting and she is afraid it can fall on her house or their neighbor. This happened in May and when she asked me for help, I went to the 1st Baptist Church of Orange in where the headquarters of the Southern Baptist Men were. Last week I encountered the lady and she told me that the tree still there and the person from the Baptist men that went to her house, never returned.
