Summary of Case Study:
Bill is a 34 year old male who has recently been prescribed analgesic and muscle relaxant for his right hip pain. After three weeks of pain he is still experiencing symptoms. However, he is now experiencing stiffness and pain in his left hip as well.
The physician has examined his x-rays and believes that he has avascular necrosis. The physician thinks this may be possible due to his have decompression sickness in the past. This can cause tissue damage due to the lack of blood supply to the bone.

Many people may assume that bones are all the same. However, bones can be classified by shape, structure, and bone markings. However, avascular necrosis is also referred to as aseptic necrosis, osteonecrosis or ischemic necrosis. Avascular necrosis most commonly affects adults between the ages of 30-60 years old. The causes are bone injury, lipid deposits in the blood, and diseases such as sickle cell anemia. Some of the symptoms that a patient with avascular necrosis may experience are joint pain. These symptoms can worsen over an in longed period of time and can occur when standing or lying down ("Diseases and Conditions: Avascular necrosis") The most targeted areas in the body for Avascular necrosis are the hip, shoulder, ankles, knee, hands, feet, the epiphysis of the femur, tibia, and the fibula ("Diseases and Conditions: Avascular necrosis"). Jimmie Bowman was seen in followup for CIDP, causing previous weakness and numbness of his distal lower extremities. He states that the strength of his distal lower extremities [____] continues improved and is staying normal. He has occasional mild feeling of numbness of his feet, but states this is staying down to what he can tolerate. He is not having pain of his feet. He is no longer on Imuran.
Percocet decreases pain by 80% and enables him to perform light gardening and household chores. No side effects with Percocet use. He is doing well with current medications and is compliant with no aberrant behavior. Upon lumbar examination, motion is associated with increase in pain. Left seated straight leg raise is slightly positive.
Based on progress report dated 05/23/14, the patient reports of continued dull aching pain and burning sensation into the cervical spine. She received 2 cc of lidocaine with no epinephrine in the bilateral trapezius, cervical rhomboid, and cervical paraspinal muscles on this visit. Based on progress report dated 07/10/15, the patient complains of unchanged, sharp, dull and aching pain in the cervical spine, which radiates to the bilateral upper extremities. Baseline is 6-7/10 pain. Rest and medications help alleviate the pain.
CURES report was reviewed. Last urine drug screen on 12/04/14 was appropriate. On examination, there is tenderness upon palpation over right medial knee joint line. Mild atrophy of right medial quadriceps muscle is noted.

As the summer wore on, I began to experience hip pain. It was only minor and I thought nothing of it. I had aches and pains all the time, and they had always gone away on their own before. This pain was different though, it didn’t go away. Cross country season rolled around and the pain was still there.
He recommended Drs. Gleiberman, Halbridge, and Suzuki. I would not recommend the use of an Agreed Medical Examiner. It does not appear that we will be able to settle this case without going to an independent doctor. I would recommend a panel qualified medical examination after the next deposition.
This week I have seen a very interesting case at the Vine Clinic. A 17-year-old female who came in with her mother had a chief complain of joint pain and a severe headache uncontrolled by Ibuprofen. She has recently seen in the ER for joint pain. I observed my preceptor conducted a thorough history and physical exam. She started joint pain on the right side of her knee and then the pain migrated to the left side of her body and affecting all the major joints.
The patient was diagnosed for polyps and multiple diverticula at the age of 68. The Patient suffers of painful osteoarthritis of both knees, shoulder hips. Patient?s mother deceased at the age of 79 from breast cancer and her father deceased at the age of 54 from heart attack. The patient noted with bilateral lower extremities edema, and claimed that she uses 2 pillows as a comfortable position to sleep,
Joints that are commonly affected are the knee and hip. • Most cases of acute septic arthritis are caused by bacteria such as Staphylococcus or Streptococcus. • Chronic septic arthritis (which is less common) is caused by organisms such as Mycobacterium tuberculosis and Candida albicans. • Once the infection reaches a joint, the joint cannot protect itself from the invading bacterium, virus, or fungus. • The body’s reaction to an infection in the joint includes inflammation, increased pressure, and reduced blood flow to the joint.
