`At Seattle Children’s Hospital, Kimberly Hiatt had been a cardiac critical care nurse for 24 years. On Setember 14, 2010, she accidently gave an already sick 8-month old baby 1.4 grams of calcium chloride instead of the prescribed 140 milligrams. She immediately realized what had happened and stated “Oh my god, I have given too much calcium”. This was only the medical mistake she had ever made. A few days later, the baby died.

What a blessing it is to have the United States Department of Veterans Affairs (VA), Veterans Health Administration (VHA) providing multiple healthcare services to our heroes, our veterans! To be afforded the opportunity for our heroes to attend and be treated, at little to no cost, for a wide range of ailments from the common cold, to a mental health disability, or for the possibility of a healing surgery is a well-deserved and a wonderful privilege. One of the major demographics of care providers within the VHA is the registered nurse. There are over 51,000 registered nurses nationwide, with 60.7% of the total population of providers of healthcare, within the VA hospitals, VA outpatient clinics and VA Patient Care Medical Homes (VHA, Office of Nursing Services, 2012).
The shortage of primary care physicians has been a problem in health care system and will continue to the next decade due to increasing of aging population. According to National Governors Association (NAG) (2012), the growth of the aging population combined with the initiative of the Affordable Care Act, the problem of primary care providers shortage is worsening. Nurse Practitioners (NPs) are educated and prepared to perform primary care services as physicians do. More importantly, studies showed that NPs’ role as primary care providers achieve equal or higher patient satisfaction rates (Evangelista et al., 2011, Albers-Heitner et al., 2013). These studies illustrates that NPs can play a key role in providing services to health care market.
The four spheres of political action in nursing are government, workplace, professional organizations, and community. They are all interrelated and interconnected. The government enacts policies that govern and affect all spheres. Healthcare policies are often made to ensure access, quality, and affordability of care. It determines the scope of nursing practice at a state level. The workplace serve a specific community and have its own policies to make the organization run efficiently and cater to the needs of the community that it serves. Workplaces follow Federal laws and regulations to ensure public safety. Nurses have to abide by the rules and regulations of their workplace as well as the government. This is also the sphere where nurses can have the most influence.
It is very important for Health programs to reach out and be able to help all sorts of different kinds of people, which are being affected by an illness or disease. Programs that know about individuals and their culture will benefit greatly, when knowing the background of their culture and why they may struggle with such problems in their health. I found two programs that are working for the people, the programs had to be beneficial with the certain cultures of people they are trying to reach out to. For one of the programs I decided to talk about was the, Black Women’s Health Impetrative and its interactions to help inform African American women the signs of Cardiovascular disease. Heritage Valley Medical Center has had a wonderful reputation for providing excellent health care services to their community. Initially, their community was 80% Caucasian, 40% African American, and 5% Hispanic. However, in the last 5 years, the population has changed to more minorities and the whites have moved out to the suburbs. This caused the Center’s occupancy rate to go down 40% because many of their traditional, more affluent, private-pay patients had left the neighborhood. To bring in revenue, they campaigned to bring in more Medicaid patients. By the third year, their patient increased from 10% to 40%. Many are African Americans and Hispanics. The staff, however doesn’t reflect the patients’ diversity. The staff is 85% Caucasian, including nurses, laboratory technologies, pharmacists, and therapists. There are two African American and one Hispanic managers. The executive management staff was 100% white and the support and administrative team were 100% Afrincan American. In a recent survey, only 30% of blacks, 10% of Hispanics and 20% of Asians were very satisfied with their services, compared to 80% of Caucasians. They said they didn’t feel welcomed and nobody understood them. The manager staff had a lot to say back, revealing subconscious racism and bias towards the minority patients, and even toward each other. The problem must be addressed.

Unfortunately this lack of understanding causes millions of Americans to suffer from heart disease. According to the Center for Disease Control (CDC) “Heart disease is the leading cause of death for people of most ethnicities in the United States, including African Americans, Hispanics, and whites.(2)” Among these ethnicities, African Americans have the highest death rate. Why is it that African Americans are at the top of this list? Perhaps it is because of the social inequality experienced on the socioeconomic scale. Socioeconomic status can heavily influence the optimal health of the heart in an individual; resulting in cardiac injury. Socioeconomic status is comprised of income, housing, education, employment, and social position.
