Introduction Ms. Jones is a 28 years old African-American female, who came in to Shadow General Clinic for a pre-employment physical. The patient is the main source of information for obtaining her history and physical. Ms. Jones was last hospitalized in Shadow General Hospital six months ago for a wound on the bottom of her right foot. Her past medical history includes asthma, which was diagnosed when she was 2½ years old, type 2 diabetes diagnosed at age 24, hypertension and PCOS (polycystic ovarian syndrome) four months ago during her gynecologist appointment. Maternal grandfather died at the age of 80 from a heart attack, he had hypertension and high cholesterol. Maternal grandmother died at age 73 due to a stroke and had hypertension and high cholesterol. Paternal uncle has history of alcoholism. Ms. Jones is in a new relationship and lives with her mother and sister but is planning to move out in a month once she has started her new job. She very involved with her church and has a strong connection with her faith. She is not sexually active and is using birth control pills. This condition is link with patients who are obese, is diabetic and has polycystic ovarian syndrome (Sander, 2016). This condition is all present with Ms. Jones. Another problem that we identified is the scattered pustule on her face which is commonly called acne. Adult women may present with acne involving lower face and neck is often associated with premenstrual flare and they seem to benefit from hormonal therapies (Thiboutot & Zaenglein, 2016). This condition is also present with Ms. Jones of which she claimed that her skin has gotten better when she started taking the birth control
A case is presented at the hospital of a 2-month-old child, diagnosed with down syndrome, and currently recovering from a case of bronchiolitis. The child, Elisa, is also diagnosed with Tetralogy of Fallot, and she will undergo surgery to correct this defect once she has had time to recover from her bronchiolitis. Tina’s paternal side consists of her grandfather, who died five years ago at the age of sixty- five due to colon cancer. He has a history of type two diabetes.
She fits this well because of her substance usage of marijuana daily. We can rule out generalized anxiety disorder for this one because she shows no signs of distress of anxiety. Also another one she fits well with is Somatic Symptom Disorder 300.82 (F45.1). She has symptoms of being in pain for more than 6 months and no one can figure out what the actual cause is. Day 7 In the middle of the night I started to have an asthma attack. Lexi woke up and told me to breath in and out. She could not for inhaler but instead of freaking me out she reminded me that all day I had not used it and that meant I can handle a night without my inhaler.
