Patient Goals: Pt’s primary goal is to improve both static and dynamic balance in order to decrease his frequency of falls. Pt wants to increase bilateral LE strength to improve his ability to perform functional tasks such as, sit to stands. Pt also requested to become educated on and provided with a HEP to improve his posture. Pt’s final goal is to increase his endurance during ambulation so he can remain independent without an AD.
Circumstance: Ayden will maintain contact with medical team monthly. Ms. Smalls (MHP) and Mrs. Wigfall (MHS) discuss Ayden’s recent medical appointments and therapy. Action: MHS report Ayden will start physical therapy at an outside clinic. MHP and MHS discuss Ayden receiving all therapy at the same clinic to reduce several therapy appointments during the week. MHP and MHS review reports given since last week.
a. Screen failures will be notified by Dr. X that they cannot participate in the pilot study because they do not meet the subject inclusion criteria. The first unit leadership and staff were involved from the very beginning and the 2nd unit received training just prior to the program being initiated. Rounds were completed every 1 hour between 0600-2200 and very two hours between 2200-0600. Falls on unit one reduced significantly. Staff voiced positive results and felt they had time to complete patient care, call light times were reduced and recommended the program be initiated on different units. On unit 2 no significant reduction in falls was noted.

Through evaluation of currently UTMB policy and recent policy guidelines, no new additions to the policy were identified. Rather, recommendations to increase compliance and ease staff load were identified. These recommendations include increased collaboration with physical therapy to increase equipment availability, early identification of therapy needs with a patient and increased compliance with daily exercise programs. As the cost of falls has been identified as billions of dollars, an economical decision should be made in order to increase supply of walkers available on the units. For goals to be useful they must be SMART, which stands for specific, measurable, achievable, relevant, and time-based. Margret Amatayakul defines SMART goals as “statements that help an organization establish expectations that are specific, measurable, achievable, relevant, and time-based” (Amatayakul, 2016). Specific, measurable, and time-based are pretty easy to understand and achievable and relevant are more abstract. A key element in setting a SMART goal is identifying “who,” “what,” “why,” “how,” and “when” will be considered in the process” (Book).

PTGoals: STG to be achieved in 4 weeks: 1) Pt will be able to perform bed mobility where he is able to roll to both the sides with minimal assistance with 25% verbal, visual and tactile cues. 2) Pt will be able to perform functional transfers from bed to w/c where he is able to assist with 75-99% of the transfers, minimal assistance with verbal, visual and tactile cues.
