On 07/15/2015, at approximately 0038 hours your affiant was on routine patrol in the area of the three hundred block of west Poplar Street. I observed a blue Chevrolet sedan parked in the road midway in the block. I proceeded patrolling approximately three blocks and observed the said vehicle in the same location. The vehicle was facing east on Poplar Street. I proceeded toward the vehicle, observing two individuals inside. The vehicle began to move stopping at the stop sign and illuminated the interior lights. The vehicle stayed in the road at the stop sign for approximately two minutes as I was directly behind them. As the vehicle turned left (North on Chestnut Street) I activated my emergency lights. The operator continued to the alley and turned left in the alley (West on Columbus St). The operator was identified as Charles URBANAVAGE. I detected a strong odor of an alcoholic beverage emitting from the vehicle as he spoke. URBANAVAGE displayed red glassy blood shot eyes, a slow slurred speech, and constricted pupils. I asked URBANAVAGE how many drinks he had tonight. URBANAVAGE stated "none, I didn 't drink". I asked why I could smell alcohol coming from his vehicle. URBANAVAGE said his passenger was drinking. The passenger was identified as Diane KUTSKO who is known from previous contacts. I requested URBANAVAGE to exit the vehicle and submit to a series of psychophysical divided attention tests; none of which he could successfully complete. At this time I advised URBANAVAGE of my findings and requested him to submit to a chemical test of blood, at the Schuylkill center medical south hospital. Driver of vehicle 1, Renneker stated she picked up four customers for a carriage ride before traveling southbound on South Leonor K Sullivan Boulevard. Renneker said she observed the bridle over the horse eyes fall off; at which, she stopped and exited the carriage to reapply them. Renneker said she advised the passnegers to exit the carriage while she was reapply the bridle. Renneker said as she was reappling the bridle a helicopter took off from the landing paid and she believed it spooked the hourse. Renneker said the house took off running southbound on South Leonor K Sullivan.
I observed a green Honda Accord traveling west on Washington Boulevard directly past our patrol unit. I saw the vehicle did not have a front license plate in violation of CVC 5200(a)-No front license plate. The vehicle continued westbound on Washington Boulevard and pulled into the northwest parking lot of the Kings Villages apartment complex. I made a U-turn, activated my overhead lights/siren and conducted a traffic stop on the vehicle in the northwest parking lot (1141 N. Fair Oaks Avenue). As we pulled in behind the vehicle, the driver (later identified as Suspect Davian Thomas) exited the above vehicle (California license plate #4GCN238).
Abdalla, while seen walking in the video, is holding an umbrella, which is tilted to her right, towards oncoming, eastbound traffic, which could have obstructed her view of Vehicle 1 traveling eastbound on Fletcher Avenue. Abdalla crossed the path of and was struck by Vehicle 1. Debra McKee was operating Vehicle 1, a 2008 Pontiac G6, eastbound on Fletcher Avenue, with a green traffic signal for eastbound traffic, at a speed of approximately 28.57 miles per hour in a 35 mile per hour posted speed zone. McKee 's view of the roadway was obscured by a very heavy rain.
S/S Folwaczny contacted Non- Emergency 911 concerning two black sedans parked on the premises in front of OPS 1 at 12490 Sunrise Valley Drive. S/S Folwaczny spoke to Dispatch Operator Dang concerning the vehicles, After running the VIN number, D/O Dang was able to confirm that the vehicles were not stolen and that any further action pertaining to the vehicles would have to take place Sprint Security.
On 01/23/2016, at approximately 1428 hours, your affiant was on routine patrol travelling north on State Route 924 (North Main Street). Your affiant observed a black Subaru station wagon travelling south on State Route 924 approaching a steady red traffic signal located at the intersection of Main and Coal Streets. The operator of the Subaru continued through the intersection and travelled through the steady red light. Your affiant activated the emergency lights and conducted a vehicle stop on the unit block of North Main Street. Your affiant approached the operator and requested his license and vehicle information.

When the officer asked Partlow for his registration, he opened the glove compartment where the officer observed a large roll of money inside the compartment, later confirmed as $763. The officer asked Partlow for consent to search the vehicle, which he consented.
10 Investigative Skills (7PA0006337) - Officer Stark responded to a PC211-Robbery radio call at 1038 E. Colorado Blvd. The comments of the call stated 2 Female Asians and 2 Male Asians pushed the Victim out of his wheelchair and took his wallet containing $25. Stark responded to the location, even though it was out of his beat, and asked me if he could handle the call. I advised the beat Officer that we would handle the incident for training. As we entered the Rite Aid, the Victim was sitting in his wheelchair directly in front of the exit.

On 04/19/16 at approximately 9:22 AM, Tpr. Mike Harris stopped a white Chevrolet Malibu (Michigan Tag # DES4556) traveling westbound on I-10 at the 4 mile marker, for following too close. Tpr. Harris made a passenger side approach to the vehicle, and asked the driver for his driver’s license and vehicle registration.
On the above date and time I was conducting an area check of the the Ridge Road Center plaza. While behind the plaza, I observed a white, 4 door Mercury occupied by three white males. The vehicle was located in the back of the plaza next to a closed store. As I passed the vehicle all four males continued to stare at me. Concerned that the driver, or passengers may have a medical emergency, or a disabled vehicle, I proceeded to pull behind the vehicle without my lights or sirens activated to inquire.
In summary, on 06/05/16 at 2254 hours Ofc. E. Vera #289 and I were patrolling the area of 5900 W Roosevelt Rd. , at which time I observed a vehicle (03 ' silver Volkswagen IL Y804576) disregard the stop light located at 59th Ave./Roosevelt Road. A traffic stop was conducted on the vehicle.
Driver involvement in personal injury or fatal motor vehicle accident; chemical test.(a) Any person who drives or is in actual control of a motor vehicle upon the public highways of this State and who has been involved in a personal injury or fatal motor vehicle accident, shall be deemed to have given consent to a breath test using a portable device as approved by the Department of State Police or to a chemical test or tests of blood, breath, or urine for the purpose of determining the content of alcohol, other drug or drugs, or intoxicating compound or compounds of such person's blood if arrested as evidenced by the issuance of a Uniform Traffic Ticket for any violation of the Illinois Vehicle Code1 or a similar provision of a local ordinance, with the exception of equipment violations contained in Chapter 12 of this Code, or similar provisions of local ordinances. The test or tests shall be administered at the direction of the arresting officer . The law enforcement agency employing the officer shall designate which of the aforesaid tests shall be administered. A urine test may be administered even after a blood or breath test or both has been administered.
An Enforcer inquiry was conducted to determine if any members of the 110 Detective Squad ran the name "Shawn Thomas" from 2100 hours on 06/08/2015 - 2100 hours on 06/09/2015. The Enforcer Check revealed that two (2) members of the 110 Squad ran the name "Shawn Thomas", Det. Kelly and Det. Patel (formerly assigned to the 110th Squad). These two officers were subsequently added to this case as subjects with the allegation of DRV-Other for failure to take appropriate actio n in regards to an active I-Card.
The suspect’s statement of the victim being drunk was confirmed in the autopsy. It was also found that the cause of death had been from a wound on the head. There was no blood found on the scene, so he must’ve died of blunt force trauma or internal bleeding, as the autopsy is not fully definitive at the moment. Americans know the dangers of getting behind the wheel while intoxicated. When alcohol enters the human body it impairs your judgement. Your speech begins to slur, the overall control you have on your body vanish because your balance and flow are altered by the alcohol entering your bloodstream. But yet the court systems lets them off with a simple slap on the wrist , like a mother to a child. What happens when that “first time offender” , loses their sense of knowledge of how to drive a car.
