On 10-14-2015, at approximately 1600 hours, I was notified by Sgt. E. Kelley, # 2154, of a traffic crash involving life threatening injuries. on Lumsden Road, east of Providence Road. The crash was reported via 911 at 1456 hours.
Upon my arrival to the scene at approximately 1645 hours, I observed the crash scene to be just east of Providence Road along the south side of Lumsden Road and at the east driveway entrance to the Brandon Centre South shopping plaza at 1945 W. Lumsden Rd, Brandon, Florida. Both vehicles came to rest off the roadway only the outside eastbound traffic lane was closed to traffic by Hillsborough County Sheriff’s Office patrol units. The driveway was closed to all pedestrian and vehicular traffic. Master Deputy Millovitch # 7044 had also responded to the scene and was originating the DHSMV Florida Crash Report (86200813).

The driver of the motorcycle, Valerie Francesca Torres, was driving east on Lumsden Road at a high rate of speed. The driver of the Durango, Angel Luis Mendez, was driving west on Lumsden Road and made a left turn across the eastbound lanes of Lumsden Road towards the driveway entrance to 1945 W. Lumsden Road. (See interview for details) The driver of the motorcycle had been transported prior to my arrival.
After interviewing Mendez, I observed no indication of impairment or involvement of drugs or alcohol on behalf of the driver. I asked Mendez if he would submit to a voluntary blood draw and he said that he would. I then transported Mendez to Hillsborough County Fire Rescue, (HCFR) Station #37 at 5602 Providence Road,
2. Police are investigating a four tractor-trailers and fourteen-car pileup that occurred on Interstate 790 at 6:45 a.m. this morning. Two people died on the scene and the accident injured twenty-four others, and four of them are in life-threatening conditions. Thibodaux Regional Medical Center 's Life Flight flew away two of the worst injured from the accident.
RECONSTRUCTION PRE-CRASH: On 05-26-2015 at approximately 2223 hours, Vehicle 1 was eastbound on Fletcher Avenue in the outside lane, just west of and approaching the intersection of 22nd Street. There was a heavy downpour of rain and the driver of Vehicle 1 did not see Pedestrian 1 crossing Fletcher Avenue. Pedestrian 1 was walking southbound across Fletcher Avenue, in or near the marked crosswalk, on the west side of 22nd Street. Pedestrian 1 was walking against the “Do Not Walk” signal, for pedestrian traffic.
Facts: Rudy Stanko was driving on the Montana State Highway 200 when he was pulled over by Officer Kenneth Breidenbach, a member of the Montana Highway Patrol. Stanko had been driving his vehicle at a steady 85 miles per hour at a location that was “narrow, had no shoulders, and was broken up by an occasional frost heave.” This location also included curves and hills which obscured vision of the roadway head. The actual roadway held no other drivers at this time during the day. Stanko had been driving his new 1996 Chevrolet Camaro, with brakes, tires, and a steering wheel that were all in perfect operating conditions.
On Friday, 10/04/17, V1 Linda West was the victim of suspected elderly abuse and assault with a deadly weapon by S1 Ricardo Sainz via being struck with a black metal stick and suffered a right broken femur. This occurred at 567 Sylvan Street #A, Daly City. V1 West was transported to Stanford hospital via an unknown means. This case will remain open for follow up information. INVESTIGATION:

WOOSTER — A Sugarcreek man who drove his buggy into oncoming traffic, causing the death of another driver, accepted his criminal responsibility when he appeared Wednesday in court. Robert S. Coblentz, 75, 2662 State Route 93, pleaded guilty in Holmes County Municipal Court to a single count of vehicular manslaughter. Joyce Morris, 74, Maple Street NW, Sugarcreek, died Jan. 20 from injuries sustained in the crash, which occurred at the intersection of state Route 39 and County Road 114 in Walnut Creek Township, according to the Wooster Post of the State Highway Patrol. In contemplating his plea, Coblentz, who appeared in a wheelchair, said, “The charge is correct.
In summary, on 06/05/16 at 2254 hours Ofc. E. Vera #289 and I were patrolling the area of 5900 W Roosevelt Rd. , at which time I observed a vehicle (03 ' silver Volkswagen IL Y804576) disregard the stop light located at 59th Ave./Roosevelt Road. A traffic stop was conducted on the vehicle.
On 12/20/2015 at 00:41 hours, while traveling southbound on Hurffville Road, I observed a Black Ford bearing NJ Z47CSR failing to maintain lane. The vehicle was swerving to the left over the center lane. I activated my overhead lights and sirens and conducted the motor vehicle stop on Rte. 42, mile marker 11.7. 2. WHEN STOPPED I approached the driver’s side window of the vehicle and spoke to the driver, Denim P. Dyczok.
Raw Copy 1 A couple was killed after a truck crashed into their car on Finkerton Highway, south of Flaxtown yesterday. The couple was travelling from Jamestown to the capital city of Flaxtown when a truck transporting bovines to a local abattoir lost control and veered onto the wrong side of the road before coming to rest. Twenty-eight bovines were also injured in the crash which cause the highway closed for about four hours.
(CNN)Children and infants made up almost a quarter of those injured when a car crashed into crowds at the Oklahoma State University parade Saturday, killing four people including a 2-year-old, Stillwater police said. The car's driver has been arrested on suspicion of driving under the influence. Three adults were pronounced dead at the scene, while the fourth victim, the young boy, died from his injuries at OU Medical Center Children's Hospital, according to a Stillwater Police Department statement. Police named the adult victims Sunday as 23-year-old Nakita Prabhakar of Edmond and 65-year-old Stillwater residents Bonnie Jean Stone and Marvin Lyle Stone.
EMS Response to Opiates As I approached the scene I saw two Suffolk County police department sector cars on scene and a first responder. This was a summer evening, you could still feel the humidity lingering in the air. Over the radio, the cops relayed that CPR was in progress. I approached the house, entered using the side entrance.
In summary, on 09/25/15 at 0159 hours I was patrolling the area of 1800 S. 59th Ct., when I observed a vehicle (02 ' Pontiac IL V707523) traveling southbound at a higher rate of speed than the posted speed limit (20mph). I paced the vehicle at 35mph for approximately for two (2) blocks (1900-2100 59th Ct.). The vehicle had an obstructed front windshield. I made contact with the driver at which time I requested a driver 's license and insurance for the vehicle. The driver was unable to provide any form of identification, but was able to provide insurance.
This is a case concerning negligence. The plaintiff, Mr. Davis’s wife, wishes to bring this case to the court under negligence law because of the death of her husband in a car accident. There are two defendants in this case. The first defendant, GM Holden Ltd, is a car manufacturer. The second defendant, Brown’s employee, is a truck driver.
While Riley was driving home from work that day, she traveling at a rate of 55 mph down the interstate when a semi truck entered the lanes on an oncoming ramp. She tried the brakes but nothing happened, a split second later she was crashing into the eighteen wheeler. She did not have time to switch lanes. The car began to crumble as it flipped over the asphalt until it came to it resting place in the grass patch between the North and South lanes of I 65. Witnesses recalling the accident trembled from fear at what they had just saw.
On the morning in question the road had several icy patches on it. The accident occurred when the van driven by Mr. Hart skidded on ice and struck Mrs. Forrester.
