R/s Vickie stated she found her mother in feces and her father locked her out of the home.
ALLEGATIONS:
R/s Phyllis Fisher is in a wheelchair and has hypertension, weakness, COPD, O2 Dependent, anemia, and a history of alcohol abuse. R/s Mrs. Fisher lives with her husband, but he can’t take care of her. R/s Mrs. Fisher has been found in her feces. R/s Mr. Fisher locked his daughter, Vickie out of the home and she can’t help her mother. R/s the family recently moved from New York and there is open APS case in New York. R/s Mrs. Fisher only takes her hypertension medication once in every three months.
On 8/1/2015 S/O EMT Perez was dispatched to FC-609 regarding a fall. S/O EMT Perez announced hispresnece and knocked at the door and was verbally greeted in by the resident. The fallen resident, a Mrs. Ida Looney stated she lost her balance and fell while trying to get up from the living room couch and was unable to get up on her own strength, Mrs. Looney was on the couch by the time S/O EMT Perez arrived. Mrs. Looney stated that; she was unaware of any change in medications nor was she aware that she was on any blood thinners, She did recall and remember the fall and was unaware and or could not recolect weather or not she was seeing a doctor here at riderwood. Mrs. Looney seemed to be a bit complacent mentally to which her spouse said was
Spoke to Vivian Ortiz (DOB 6/20/75) who stated that her juvenile son (16 years old), Elijah Perez (DOB 7/3/99), was being destructive and threw things around the house when she asked him to turn off the oven after he used it. Ortiz stated Perez had been diagnosed with intermittent explosive disorder and he had not been taking his medication or to his therapy. Ortiz also mentioned that she opened the case with DYFS to evaluate him and the case was still opened. Ortiz stated that Perez threw some baseball cards, laundry bin, trashes and made mess inside of the refrigerator. We weren’t able to speak with Perez as he walked out of the house prior to our arrival.
On 7/20/2015 S/O EMT Perez was dispatched to MG-601 regarding a fall. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/ O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor.
On 9/25/2015 SO EMT Perez was dispatched to VP-113 regaurding a fall. S/O EMT Perez knocked and announced his presence at the door and was greeted by the residents spouse and introduced to the fallen female resident a, Mrs. Dorothy Gish who was sitting upright on the living room floor by her rollater. Mrs. Dorothy Gish stated that she was sitting on her rollator when she slid onto the floor and was unable to pick herself up on her own strength. S/O EMT Perez performed a pelvic exam on Mrs. Gish which revealed no pain or grimace.
Roberta said on 06/20/2017 Remington called and said he was on his way to Roberta 's home. Roberta said Remington was going to her home because nobody to answer his calls. Roberta said Remington sent a picture of the back porch of her home. Roberta said Remington did not have permission to be at the home. Roberta said a family member went to her home and noticed the door to the kitchen
R/s Sandra told Anna she was going to call DSS and said Anna couldn’t come back home. R/s Anna recently moved back with her mother due to having multiple surgeries on her mouth. R/s Anna is afraid she doesn’t have anywhere to go this evening. R/s Anna also reported that one her mother mushed her in the head.
Lisa do not take Jackson away from Bonnie and Donald’s home; she’s aware that it’s not safe for Jackson to be around the drug use. Lisa has stolen things from Bonnie and Donald’s home, but has forgotten her behaviors. Lisa has threatened to take Jackson away from Bonnie and Donald, which has prevented Bonnie from reporting the allegations previously. Within the past 6-8 months things has gotten worse with
R/s Ms. Hardee was just released from the hospital on Monday after being there 6 weeks. R/s Ms. Hardee isn’t able to cook nor bathe herself. R/s Ms. Hardee has a little trouble with
On 7/9/15 worker made an unannounced visit to the residence of Ms. Bernice Connell, for the purpose of making first victim contact. Ms. Kayley McKinnon, granddaughter-in-law of Ms. Connell greeted worker at the door and showed worker to Ms. Connell 's room. The room was cluttered but did not present with an odor. Ms. Connell was lying in bed watching TV, she was appropriately dressed with good personal hygiene. Ms. Connell stated she had lived with her son, Earnest McKinnon, and daughter-in-law, Arlinda McKinnon, for two years.
SC placed call to Pa and spoke with Bill Pa’s spouse who reported that doing ok. But she has come combative lately and verbal abusive. Bill spoke at length about her behavior and the impact it has on him. Bill is offended when Pa makes disparaging remarks about him and calls him out of his name. Bill reported that he believes Pa needs a medication review and a change in meds.
Medication issue: The CP addressed with Mrs. Navarro about the CP conversation with Dr. Freddie Marton with regards to Tyler’s medication regimen. The CP asked Mrs. Navarro if she is providing Tyler’s with his medication and her response was that she needs to do a refill and then she says, “Oh, I cannot find them since we are packing.” However, Tyler intervenes and stated that he knows where the medication is and based on Mrs. Navarro reaction, she appeared stunned. Tyler left the living room and returned with the bottle at which appears to be full. The CP glazed at Mrs. Navarro reaction and she immediately stated that she will start giving him his medication and will also considered the school nurse providing Tyler with his medication at the school.
Davis said that approximately three years ago Mr. Davis suffered a stroke and lost use of a good portion of the left side of his body. Mrs. Davis said that Mr. Davis' condition has been deteriorating and last week Mr. Davis had an appendectomy complicating Mr. Davis' condition. Mrs. Davis said this morning Mr. Davis began complaining he was constipated. Mrs. Davis said she helped Mr. Davis to the restroom where he spent approximately 20 minutes and she then helped him back to the bed. Mrs. Davis said Mr. Davis told her at this time that he "could not take this anymore" possibly referring to his physical condition.
he child's maternal grandmother stated Anna has a history of domestic violence, drug use, and suicidal ideation. The reporter stated Anna has been diagnosed as bipolar and is currently on suicide watch by local law enforcement. Tamara stated Anna was recently released from jail and had plans to spend time with the victim and the reporter while she gets back on track and pick up her medications. Anna left the home on 10/20/15 and has not returned and the reporter has received text messages from Anna stating plans to take her life and heard from others that at this time Anna may be suicidal and plans to come pick up Addyson. Tamara stated she's had custody of Addyson all her life and contacted her lawyer; Ms. Wright's lawyer told her there was
This week at coastal hospice I felt more comfortable with the staff and was able to address some of the concerns without hesitation. The only thing I still do not like is how I have not been able to keep one consistent preceptor during my rotation. This would not be an issue if I was being trained to work there. I think it is a great idea to learn from multiple nurses and get an insight of the way everyone does something differently.
Jonathan and Brittany Harris took their 23 year old daughter to a cabin in North Carolina for a vacation on break from college. Their daughter Jessica had a bedroom downstairs in the cabin and was preparing a bath and going to drink a glass of wine. A man broke into the cabin and went into the bedroom of Jessica’s where she was at and she bolted to
