Reason for Referral: Michael reports he is considering a change in employment but is unsure if this is a good idea at this time. He reports feeling tired and frustrated on a consistent basis.
I. Current Life Status:
Occupation: Michael has worked in retail for the past six years at Sears. He was promoted to a management position after six months of employment. Prior to working at Sears, Michael had several retail and sales positions, each one lasting less than a year.
Education: Michael graduated high school in 1989. He has completed numerous training programs through his employer. He reports he considered taking college classes in the past but chose not to due to family obligations.
Family: Michael is the oldest of two children. His parents divorced when he was three years old. His mom remarried when he was 6 years old. Michael states he has not seen his biological father since he was 18 years old. He reports being close to his brother, mom and stepdad. Michael was previously married for 5 years. He and his ex-wife have a son, who is 23 years old. Michael re-married 15 years ago. He and his current wife had a son who passed away.

He reports numerous medical issues. He was diagnosed with asthma as a child, however, this is no longer a concern. In 2004, he was diagnosed with Thyroid Disease and placed on medication. In 2005, he was diagnosed with Cavernous Hemangioma, a genetic condition where lesions form on the blood vessels. The treatment for this condition is surgery to remove the angiomas if they become problematic. Michael reports having approximately 1000 angiomas in his brain, which are monitored through an annual MRI. He was diagnosed with Celiac Disease in 2011, which is controlled by diet. Michael is currently undergoing a battery of tests for pain in his right hip. No specified issue has been
