On 7/20/2015 S/O EMT Perez was dispatched to MG-601 regarding a fall. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor. Mrs.Cotts stated had no injuries but wished to be lifted up back on her motorized scooter. S/O EMT Perez performed a pelvic exam which presented with no pain or grimace on Mrs.Cotts. S/O EMT Perez knew that he couldnt safely lift Mrs.Cotts by himself so he called S/O EMT Tucker for assistance. S/O EMT Perez and S/O EMT Tucker safely lifted
An emergency call comes in at 9:45 am from Doug Greene. He discloses that his neighbor and recently ended love interest, Anna Garcia, a well developed thirty-eight-year-old hispanic has been skipping routinely morning walks and that the last time he saw her was at 6:30 am on August 13th. Since her dogs were unusually barking, it sparked concern in Greene. The call brought police to Garcia’s home who found her dead lying face down on her entry hallway at 9:56 am. It is 10:20 am on August 14th when police are securing the crime scene.
On 1/20/2015 SO EMT Perez was dispatched to KC-304 regarding skin pain. SO EMT Perez knocked, announced his presence and was verbally invited in by the resident. The resident, a Mrs. Marian Fox stated that she has been having some severe skin irritation the last couple of hours and she wanted a second opinion on what she should do. SO EMT Perez performed an assesment which revealed the following; Blood Pressure 120/80, Pulse 64, no visible lesions or abrasions of the area where Mrs. Fox was complaining of pain and no other pain outside the ordinary. Mrs. Fox than decided to lay back in her bed.

Gurniack, Lt. Kunak heard yelling and screaming from the cell area. Lt. Kunak went to the cell area and observed P throwing himself against the cell and spitting. Lt. Kunak got a taser and told P to calm down or he will be taser. Lt. Kuak opened the cell door and tasered P. An ambulance was called and P was taken to the hospital.
On 01-07-16 at about 0955 hrs., R/P Dominguez, noticed that Chavez was missing from the facility. R/P Dominguez advised me that Chavez has been reported missing on several occasions (under case #15001276, 15012422, 15013228). 3. Facts: 10/28/01 – Leonard Melphi collapsed in his hotel room and is taken by EMS to Mount Sinai Hospital. The EMS collected his DOB, SSN, address, etc. and also listed his friend Joann Tedesco as his next of kin, together with her telephone number. He later died that evening. His death certificate stated his name and age; however, it did not include any identifying information and omitted Ms. Tedesco’s contact information.
On 12/09/2015, at approximately 1518 hours, this Officer received a call via radio and directed to Main and Lloyd Street for a pedestrian who was struck by a vehicle. This officer and Officer Brown responded to the scene. When Officers arrived on scene I observed a 93 year old female lying in the middle of Main Street (State Route 924). The female was later identified as Agnes E. Ferguson (DOB: 7/24/22) who resides at 37 West Coal Street Shenandoah, PA 17976. A silver Subaru was stationary in the northbound lane of Main Street (SR 924) near Agnes.

Pete Hernandez was a Mexican American agricultural worker, who was accused for the murder of Joe Espinosa in Edna, Texas in Jackson County (author,year). 10/29/2015 at approximately 1918 hours, this officer was dispatched to 207 Florida Avenue for a mother and daughter domestic. Both Denise and Shirley HAZLETT could be heard screaming while I was outside the home on the sidewalk. This altercation was occurring during Shenandoah Height's trick or treat event with numerous neighbors, parents and children walking around the home. I briefly spoke with several neighbors who were standing outside. They expressed thier concerns about Denise and Shirley HAZLETT.
Officer Perez reports being dispatched to a disturbance at 11 Langton. Perez arrived on scene and met with Ann Mcelroy that stated she lost her phone and found that 2 kids had possession of the phone and was able to get the phone returned to her. Mcelroy stated she wanted to make a report of her phones information had been erased and was concerned of any personal information that was possibly had been taking prior to her getting her phone back. Perez had Mcelroy complete a witness statement form.
Before I even step foot in the door I could hear sobbing and the distress in a woman 's voice. I carried in the necessary equipment, the cops were actively doing CPR as the advanced life support provider (ALS) was looking for IV access. As EMS personnel we took over for the cops, we placed a LUCAS device on him, it 's a device that does compressions for you. Then, we continued ventilating the patient and placed him on a backboard in preparation to transport and move the patient.
On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment. On 11/27/16 CM went to conduct the Safe Sleep inspection with the DHS Compliance Analyst Jeanette Chirico, while in the second floor we went to unit #222 assigned to Ms. Tiana Footman and her three children.

I have had to face some tough adversity in my path. I finally made it though, despite all the setbacks, loses, and failures.
