This supplement is a record of my actions in cases 16-125169 (April10, 2016) and 16-125698 (April 12, 2016).
On April 12, 2016 I was assigned case 16-125169 which involves Ms. Mya Navia a fourteen year old Francis Hammond School Student. On April 13, 2016, I was assigned a second case which was 16-125698 involving the same juvenile. On April 13, 2016 I touched base with Arrosa Kanwai (mother) who had previously denied me access to her daughter (16-119375). I explained the purpose of my detail and explained to Ms. Kanwai the importance of speaking to Mya. Ms. Kanwai granted me access to both her kids as her son had also been leaving home. On April 15, 2016, I responded to Francis Hammond School and spoke to Ms. Kidist Square Mya’s school counselor. Ms. Square pulled Mya Navia from class and was also present while I spoke to Mya

Mya told me that she left home as she was suspended she admitted that she was gone for hours and had been hanging out with Amy Navarro who resides on North Armistead Street. Mya does not have any medical issues of any kind. Mya told me that no one touched her inappropriately, forced her to do anything and was not a victim of sexual assault. Mya told me that she is not afraid of being home and identified both her mother and brother (Arman) as the people she reaches out to. Mya told me that she does not consume alcohol or partake in any type of drug use. Mya does not associate or know any gang members.
It should be noted that when Mya answered the alcohol consumption question although she said no, her demeanor changed and appeared to smile when answering the question. I suspect this statement to have been false. I spoke to Mr. Don Earlington who told me that Mya has a scheduled hearing for a CHINS hearing for missing school. He confirmed that no paper work need as the school has taken action.
Case status
On 03-07-16 at about 1839 hours, Officer Saunders #7720 and I responded to 610 N. Oakland Avenue #5 for a battery with great bodily injury investigation. Upon our arrival we met with Victim Raun Gibson who stated his daughter, Suspect Lanaye Gibson hit him several times across his face and bite him on his right forearm. Victim Gibson sustained two bite marks across his right forearm as a result of Suspect Gibson bitting him. I saw that Victim Gibson sustained two bite marks across his upper right forearm. Officer Saunders and I arrested Suspect Gibson for PC 243(d)-Battery with great bodily injury.
On November 16, 2015, I spoke to Lidia Hernandez (mother) who told me that her daughter was not following the rules of the home. Lidia wanted me to speak to her daughter, although she confirmed that her daughter had been behaving better. On November 19, 2015, I responded to Minnie Howard School and spoke to Officer Hill. Officer Hill reported that Lidia Hernandez had been encouraged to report as Evelyn had been gone for 2 days.
When the child told the adult neighbor what happened, she called Mrs. Bryant; Mrs. Bryant told the neighbor she didn’t have time for that and hung the phone up. The reporter stated Lakirah called her grand father and he came to pick her up. Ms. Cullop stated that Lakirah stayed with her grandfather for the night and when her mother got off this morning she went to pick the child up. The reporter stated when the child and her mother got home, Mrs. Bryant whipped her with the same strap and Mr. Bryant, punched her down stairs, and threw a metal trash can at her. Ms. Cullop stated law enforcement was called to the school to take the child’s statement and they have also taken pictures of Lakirah’s bruises.
Mr. Earlington was familiar with Kyree Dobyns and his mother. There were no prior incidents involving this subject. I called Keisha Dobyns (mother) and spoke to her by phone. I confirmed that Kyree was late coming home on October 15, 2015. I asked Keisha Dobyns if she confirmed that her son was in fact at work on October 15, 2015.
This supplement is a record of my actions in this investigation on cases 15-175694 and 15-176587. On November 16, 2015, 2015, I was assigned a third case involving Michelle Paola Vasquez Cruz. Michelle is a thirteen year old George Washington School student. I contacted Mr. Augustin Vasquez (father) who reported his daughter had been gone for six days and has yet to return home. I agreed to follow up with Mr. Vasquez at home later in the morning.
DOI: 07/17/2013. This is a 25-year-old female cashier who incurred injury to her low back when she missed a step and fell off a ladder while stocking sleeping bags. MRI of the lumbar spine dated 10/03/2013 revealed broad based central disc protrusion at L4-L5; moderate discogenic edema along endplates at L4-5; and broad bulge with a central annual tear at L5-S1. CT scan of the lumbar spine dated 01/08/2014 revealed that at L3, bilateral pars interarticularis defects are seen with sclerotic margins. The vertebrae at L3-4 are normal in present on the prior MRI.
The caseworker’s statements are likely to be admissible under the Federal Rule of Evidence 803. Hearsay is a statement that the declarant makes at a time other than while testifying at the current trial or hearing (i.e., an out-of-court statement) that is offered to prove the truth of the matter asserted. Fed. R. Evid.
Discussion Post Week Eleven NURS6551, N-6 As an advanced practice nurse (APN), one will evaluate many patients with musculoskeletal and endocrine conditions. Therefore, the clinician must be aware of subtle differences that occur in various diseases to ensure proper diagnosis and treatment. For the purpose of this week’s discussion, I will choose a case study and explain the likely diagnosis along with the differentials.
In studying Registered Nursing one will find that they have to be compassionate and want to care for others. Registered Nurses assist physicians with suffering patients of any medical condition. They help to decide the patient’s medicine, treatment, recovery, and educate the them and their families on post-medical treatment. They need to have bedside manner, and be able to prepare patients for surgery. Registered Nursing careers range anywhere from critical care, oncology, mental health, to an everyday school nurse.
My interviewee is a fifty-three-year-old, white female named Kat Smith. Kat is the mother of two daughters and has been married to her husband for twenty-five years. She is a stay at home mom and lives a comfortable middle class lifestyle. Kat was diagnosed with at age forty-nine with uterine fibroids that resulted in a total hysterectomy. Previously Kat had lived in extreme pain for over fifteen years, she expected that something was wrong, however, the doctors that were treating Kat did not order an ultrasound and attributed her pain to her menstrual cycle.
CM conducted a face to face meeting at the Jersey City Municipal Court for Makaryos Mehany (youth) Truancy court hearing. In attendance were Jasmine Alexander (CM), Refeat Mosa (caregiver), Makaryos (youth) and Jim Warner (IIC). Youth reported that he stepped on a nail while at a relative home on Sunday, 2/26/17. Youth came to the court with one shoe due to this incident and explained to the team that his foot is swollen and he is unable to wear a shoe. CM was informed youth did not go to the hospital; CM encouraged the family to go to the hospital to check youth’s foot.
First, it is important to define what it means to be an “Advanced Practice Nurse” in the state of Indiana before we can look at the stipulations set. It is defined as a registered nurse who is currently holding: knowledge acquired from a formal, and organized institution of learning, and additionally, clinical experience, or a stated board approved equivalent.
She was manipulative and dishonest in treatment sessions as she would report going to school, but later found out that she skipped school to go to Walmart with her friends. Court documents state that secure placement was needed due to MS-13 gang member have made a threat to her life in the community. She also exhibited difficulty with inability to comply with probation, home, school and community rules.
On this date of March 19, 2005 at the time of 12 a.m., I Forensic Technician Meggan Rodriguez was called to process the crime scene of Jessica Lunsford at 6647 W. Snowbird Court Homosassa, Florida. I collected the names & identification numbers of all the personnel located at the scene then I completed a briefing with Sheriff Jeff Dawsy (the lead detective in this case), who made me aware of the missing 9 yr. old Jessica Lunsford was kidnapped from her house at the location of 7266 S. Sonata BLVD Homosassa, Florida (across the street) on the date of Feb. 24, 2005. The perpetrator a registered pedophile name of John Couey admitted to getting high on crack then he kidnapped Jessica from her residence promising to take her to her father.
