G-CM met with the client to fill out a Skid Row Housing application, and give directions to the nearest social security office. I-CM helped client fill out a five page application for Skid Row Housing. CM gave the client the closest address to the social security office, because the application requires a copy of the client's social security card. CM also advised the client to continue attending different group meetings to help with his social skills and cope with his depression. CM continued to assess client's MH and medication. R- Client presented well groomed and with good hygiene. The client also made good eye contact, was cooperative, and open for discussion. The client stated that he was having a "good" day. Circumstance: Ms. Smalls (MHP), Mrs. Gailliard (MHS), Clarence and Ms. Elizabeth Strong (DSS Worker) schedule medical appointment with the MUSC Foster Care Clinic. Action: MHP called Tara Peevy, RN at the MUSC Foster Care Clinic after MHS explained leaving several messages. Ms. Strong explain emailing the referral form to the clinic. Machelle Green explain receieing the referral form, however unable to reach the DSS worker for additional infromaiton.
Goal: Cm met with client for weekly follow up I: CM greeted client and followed up on last week’s session. CM reintegrated goal of the Harm Reduction program was to provided a referral for housing. Intern discussed plan of action to help manage the Client’s access to VA benefits and referral for VASH. Intern validated the progress the client is making and his interest in doing well.
I: CM guided client through ISP goals. CM inquired about client’s upcoming LAMP VASH appointments. CM discussed and encouraged client to get his driver’s license. CM praised client for being honest regarding drug use but also encourage sobriety. CM administered Beck Depression Inventory and review results.
Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
Housing: Santa remains challenged at filling out envelopes and following basic concepts of housing. She needs constant supervision and guidance. Case manager is responsible for filling out and sending out request for applications and client is responsible for signing off on application providing documentation. Case manager with the aid of an interpreter will inform client of all housing matters.
On 10/24/2015, CM met with the client to complete Bi-Weekly ILP Review. Client was able to come to the 3rd floor social service. In the meeting client was dressed in black floral skirt and beige sweeter and jacket. Client appears to be happy she was humming. She appears to be cooperative and friendly.
She said she thinks she was on drugs at the time. Ms. Morgan has a valid case with the agency dated 7/18/2015 for Dependency. Ms. Morgan is diagnosed with Bipolar and Schizophrenia. Ms. Morgan is not taking medication at this time and she is not receiving mental health care. Ms. Morgan is supposed to receive care through Central City Mental Health Clinic, but missed her appointment and never rescheduled.
MHP took the member home once his prescription was filed. The member states he forget to make follow up appointment with the doctor for next month. MHP told the member she will schedule follow up appointment and will let him know the date and time. The member report that he still want to find a senior citizen apartment. MHP told the member that he will be responsible for paying for the first month rent and security deposit.
Summary Recently a group of therapist administered the SAFER v. 3 to assess MM’s safety in her two-story home in Blue Bell, PA. After completing the assessment it was determined that MM has several areas that were deemed either “mild” or “moderate” problem areas. Much of this has to do with her intellectual disabilities or her mild CP, which causes her to toe walk. Of note, however, is the face that MM lives with her two parents and has an aide that comes to her home four times a week.
A New Leaf provides various different services including domestic violence shelters, after school care, and residential care. The Larry Simons Residences (LSR) is a Residential Treatment Center (RTC) for adolescent males and females ages 12-18. LSR follows regulations established by the Arizona Department of Health Services and the Bureau of Residential Facilities (Arizona Department of Health Services, n.d.). Social policies concerning adolescent substance abuse, affect this case. For example, the Patient Protection and Affordable Care Act of 2010 requires coverage for mental health and substance use disorder services (Anthony, Jenson, and Howard, 2016).

Poverty is a great issue in California that people seem to overlook such a great problem.
The Client: Carla Washburn Client Assessment Carla Washburn is a 74-year-old female. Her husband …has been married for 45 years before his death 10 years ago. Washburn is currently residing in Plainville…and has retired at the top paper mill that is currently in operation. Her son’s name is Jr and her grandson’s name is Jr Jr whom are currently deceased from actively serving their country. She currently lives alone and is reluctant of social services and agencies support.
