This is a personal statement for my application to the Family Nurse Practitioner Concentration in the Ball State School of Nursing. My personal statement provides details on my personal background and the unique impact that Family Nurse Practitioners have had on my life. I describe my undergraduate studies and how I firsthand gained interest as well as insight on the skills of a Family Nurse Practitioner. I go into detail about my aspiration to be a Family Nurse Practitioner to directly affect the patient care of a variety of patient populations. I have been working in the care industry for the past three years. During these three years I 've had the opportunity to work within different care settings, from the elderly, learning disability, challenging behaviours and supported live in to name a few. Throughout this period I managed to obtain countless caring skills working in a community and hospital setting. I shadowed the Assisted Living Manager, Coley Rainbolt. As the assisted living manager, Coley has to complete monthly, quarterly, 6-month, and annual assessments for the assisted living residents on time. She has to schedule and lead care conferences in relation to residents’ needs and family expectations. She updates care plans and arranges all the resident care needs into task specific assignments for care associates. Along with all the residents she provides for, she also manages all assisted living care associates which consists of completing interim and annual evaluations timely and addressing performance concerns.
2. Work Experiences - including dates 6/15 - current I have been working for Maxim Healthcare Services for two years. I am a home healthcare aide working with the disabled community. My plan of care includes but is not limited to assistance with dressing, shopping, performing basic hygiene techniques (bathing, brushing teeth, assisting with restroom activities, etc).

Introduction The significance of person centered planning is paramount in social growth. This assignment will discuss the role of ID nurses in person centered planning. The detailed discussion will be facilitated through literature review from studies conducted by various scholars and experts. Description: In week 7 we had visited Wesburn Manor, the Long-term Care (Clinical Placement) setting. It was our first time there, therefore as a group, we oriented the place. At this time, we went to each nursing station on each floor and introduced ourselves from the organization we were from and how we will be providing patient care to the clients in this setting. We were educated by our clinical instructor on the different access codes in the building, the policies and guidelines, our assigned floor and the appointed personal support worker. Our role of professionalism as a nursing student was represented as we came prepared and greeted each healthcare and staff member.
This portfolio represents the professional and academical achievements of my educational career in the course over the years. It is the compilation of the hard work I have completed and my accomplishments that illustrate my strengths, problem-solving ability, work ethic, responsibility and moral and professional values. Social work is a profession that dedicates its efforts to ensure the well-being of individuals and the well-being of the society as a whole. The primary mission of social workers is to meet the fundamental needs of every person, especially the ones with special needs such as those who are oppressed, vulnerable and the people living in poverty. As a social worker, I intend to use these core values such as service, integrity, and dignity as guidelines to my work to make a difference in the lives of as many needy people as I can. My main goal will be to offer services to needy people to help them solve and overcome social problems that they encounter each day in their lives.
Prior to beginning this volunteer experience, I believed that I had a number of personal attributes and skills that fit this role such as having the necessary education about people, specifically older people if to work with elderly. As part of my college course, I have studied and learned about the development of people from conception to death, including how elderly people may act, think and perceive. All the knowledge I’ve attained from the lectures definitely came in handy whilst working in the nursing home. The ability to deal with mental illness properly is important when working in nursing homes as many people in nursing homes are there due to some kind of mental illness such as dementia and Alzheimer’s etc. Personal skills such as compassion, patience, being helpful, motivated, having good communication skills and being a good listener too.
