Now, I’m going to briefly explain Directed exchange and Consumer-Mediated Exchange. Since, I want to focus on Health Information Exchange in hospitals and urgent care centers, My main focus will be Query-based exchange but, I feel it is very important to have a thorough understanding of all three ways health care information is currently exchanged. Health information is currently either by Directed exchange, Query-based exchange or Consumer-Mediated Exchange. Directed Exchange is used by providers to easily and securely send patient information such as, laboratory orders/results, patient referrals, prescriptions discharge summaries, etcetera directly to another health care professional. This information is sent over the internet in an encrypted, secure, and reliable way between health care professionals who already know and trust each other. …show more content…

It eliminated a lot of strenuous work for me. Consumer-Mediated Exchange provides patients with access to their health information, allowing them to manage their health care online in a way that is comparable to how they might manage their finances through online banking. When in control of their own health information, patients can actively participate in their care coordination by, identifying and correcting wrong or missing health information, identifying and correcting incorrect billing information, tracking and monitoring their own health. I also was able to experience this type of exchange for myself when I needed a letter for something my doctor has previously typed up, I was able to log into my chart and print it for myself. Instead of waiting two weeks like I use to have to do before Health Information Exchange. Query-based exchange is used by providers to search and discover accessible clinical sources on a patient.
