Reporter stated she spoke with the grandmother and the issue seems to be the mother contacts the grandmother to ask if the children can get off there and when they do mom never comes to get them. Mom will tell the grandmother she will be there in a little while. The children stay at the grandmother 's home on and off. 08/05/15 MSW made a re-visit with Pt and caregiver/friend. CG stated Darel from HEAP in Needles did not contact or make a visit to inspect the house, even after he told the MSW he would. Pt is stable now, prior to having a fall last week and being admitted to the hospital. CG is now back from a trip to Washington for family affairs. MSW spent an hour contacting the Community Action Partership of San Bernardino County, HEAP of Needles to get ahold of Darel and every weatherization agency in the county for assistance, but no one was answering their phones.
PIA SW interviewed Anitra Booker at the BCPD. Documentation reviewed. This interview was recorded and will be uploaded in ECF. Anitra informed PIA SW that her CPS has been closed. PIA SW explained to her that the reason it was closed was because she had informed the worker that she was moving out of State.

Housing: Client 's exit strategy is to move by 2/18/16. Client must maintain employment and submit pay stubs weekly .Client is expected to comply with savings and have saved enough for one month 's rent and security by 12/18/16, Client is to seek for housing and obtain housing by 1/18/16.Client was reminded she must meet with her Housing Specialist Mr. Brown on a bi-weekly basis, comply with all housing requirements and explore all available housing options. Social Service: Client must meet bi-weekly with Case Manager for ILP Review appointments.
Joey, I went to Credit Advisors, the non-profit organization, and I talk to them personally. They said that I have to meet their manger, Eleanor Hohman, and she is not available today. I might visit them tomorrow or Wednesday again, because I have a hospital appointment tomorrow from 11am to 2pm. On 01/26/2016 client Akia Thomas came to the SSO to meet with DOE to obtain weekly metro cards. HS De La Torre approached to the client and kindly asked Ms. Thomas if she is able to stay in order to complete a Housing Plan. Ms. Thomas stayed and the following was discussed during meeting. Ms. Thomas informed HS that last permanent address was located at 155-17 116 St. Jamaica Ave. Queens, NY from Jan, 2012 to 07/10/2014. Client left the apartment because grandfather passed away and ACS was called and they found the apartment no suitable for the family.

The reporting party (RP) stated that her 9 month old daughter (name not provided) was in the care of Maria Chavez. The RP stated she lives across the street for the home and observed the licensee removing furniture. The RP stated she questioned the licensee regarding the removal of the furniture.
Successful CC =1 CM received a call from Katlyn (Clinician at EMYL) informing CM that Tavian will have a new clinician at the end of October. Katlyn stated she will keep CM updated on the new clinician at a later date. CM was informed that Tavian is has been involved in gang affiliation and Katlyn does not agree with Tavian going home on passes. Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
According to George Anthony, Casey Anthony’s father, Casey left the home where she, her father, mother, and Caylee all lived on June 16th 2008, and did not return for 31 days, taking her daughter with her. Cindy, Casey’s mother asked repeatedly during that month to see Caylee, but Casey deflected each time, claiming she was busy with work, or that Caylee was with a nannyby the name of Zenaida "Zanny" Fernandez-Gonzalez. On July 13 2008, Cindy and George Anthony were notified that Casey’s car was in a tow yard. When Mr Anthony picked up the car both he and the tow yard employee noted a significant odor coming from the trunk. Both would state that they believed the odor to be the result of a decomposing body.

The patient to do the session today since she 's already present and waited for this counselor. This writer agreed to conduct the session. Reports stable on her dose and deny the need for a dose decrease as she denies any cravings/withdrawals.
I believe everyone on this email thread was aware of my meeting today with Joe Baldwin, Guardian, of Kathy Rennich to discuss her recent return from inpatient rehabilitation at a local nursing facility and her expressed desire to move to the Hensgen Home. Basically, in February 2017, Kathy fell resulting in a fractured tail bone. She received inpatient rehabilitation at Care Springs for fourteen days and has returned home with PT services. Since her return home, Kathy has refused to participate in ADL’s (which isn’t a change in pattern as she refused prior to the nf stay) and is demanding that she have the opportunity to live at the Hensgen Home. Kathy’s reasoning behind wanting to move the Hengsen Home isn’t exactly clear to the team.
MHP took the member home once his prescription was filed. The member states he forget to make follow up appointment with the doctor for next month. MHP told the member she will schedule follow up appointment and will let him know the date and time. The member report that he still want to find a senior citizen apartment. MHP told the member that he will be responsible for paying for the first month rent and security deposit.
On November 16, 2015, I spoke to Lidia Hernandez (mother) who told me that her daughter was not following the rules of the home. Lidia wanted me to speak to her daughter, although she confirmed that her daughter had been behaving better. On November 19, 2015, I responded to Minnie Howard School and spoke to Officer Hill. Officer Hill reported that Lidia Hernandez had been encouraged to report as Evelyn had been gone for 2 days. Officer Hill reported that Evelyn is currently under suspension for an incident involving another student.
