Good Morning, I am writing you to bring to your attention an issue UHC is having with Morning Side Nursing Home located in Bronx, NY. The following members are enrolled in our UHC Personal Assist MLTC, authorized for long term custodial care. The facility has not been cooperating in submitting the conversion documentation to HRA for chronic care eligibility determination. I have contacted the facility’s Medicaid Coordinator Andrea Gurango since November of 2016 regarding member Kamrun Nehar and made arrangements to follow up with her in a few weeks on the status of the conversion application. From November till now I have called and sent emails to Andrea requesting status.
On 3/29/2016 I spoke with Leahanne Garcia at the operation. Leahanne stated she has been employed at the operation since May 2015 as a Teacher assistant. Leahanne stated on Wednesday (3/23) that the children had just woken up from nap and Andi was leading circle time, while she was moving the block center shelf back to its position. She stated the children had started to get up from the rug for center time when she heard Andi yelling "get up, get up" and was yanking Isiah up by his arm. Andi then started telling him "you will listen to me" while pointing a finger in his face.
After summarizing a little bit about her family life and accomplishments we got into the discussion of World War II. She told me that she was just a small child around the years World War II began. For most of the duration of the war her age ranged from age six to eleven. I asked her where she was when she heard about the news on the Pearl Harbor incident in 1941. The day she heard about Pearl Harbor she was at her father’s barber shop on a Monday afternoon with her two younger sisters.

I would like to take a moment to talk about the two most important and influential people in my life my parents Sabrina and Darren Hardnick. As I sat them down to talk to them about their past and their future I actually learned a lot more about them. It’s true when they say “you learn something new everyday”. Let’s start off with my mother Sabrina Hardnick, well let’s start when she was Sabrina McIntosh. February 14,1975 at Kaiser Hospital (no longer open) in Cleveland, Ohio Sabrina Lattice McIntosh was born.
While interviewing Anna, I learned many interesting things about her family life, school activities, and hobbies. The first topic Anna and I discussed was her family life. Anna is a 16 year old junior who attends Hempfield Area Senior High School. She was born on March 17, 1999 in Mt. Pleasant PA. She lived there until her parents got a divorce.

It looks like Lila is concerned about her sister Shane who is in an adult foster home that is not a safe place for her sister
People could only imagine knowing Miss Soar on a personal level, yet here I was about to spend a full two weeks by her side in a classroom. Although I knew of Miss Soar, we never had a conversation, but that would soon change. Sitting at my study writing a shopping list my mother had asked for when I heard the doorbell ring followed by a brief conversation. It was Miss Soar! At dinner later that night my mother broke the new, “Honey, Miss Soar has requested your help at her camp this summer.” she said.
Kane is eighteen years and lives with his mother Jane. They live in a home in Loong Land. When Kane was only ten his father on a detective mission got into an accident. Kane became depressed and began seeing his father a year later at the age eleven. His mother tried to get him checked to see if there was a problem with the child, but the doctor kept saying there isn’t any and seen things from losing his father.
CCIB received a SCAR/CPS referral #0306-4829-5973-3020047. The reporting party (RP) Ashley Shinn reports her son, Aedan Shinn (DOB: 01/10/12), attends the Reyes family Day Care Monday thru Friday. RP reports a couple of months ago, her son attempted to tell her how the licensee disciplines the children when they are not asleep during their nap time. RP states Aedan at first was going to tell her, but then said he would get in trouble for being bad. As recently as yesterday, Aedan finally disclosed to the RP that licensee when checking in on the children taking a nap and notices the children not sleep with their eyes closed she will hit them on top of the head with a closed fist.
The reporting party (RP) stated the foster mother Candace Coleman gave a seven day notice on foster child Joshua Laney DOB: 3/22/07, DOP: 5/12/16 after meeting with the child 's biological father on 6/17/16 (name of father not provided). The RP stated on 6/22/16 the foster mother brought the child to the DCFS office alone with all his belonging and informed the RP that she was going out of town. The foster mother informed the RP the child could return to the home if other placement was unavailable. The child was placed in at Leroy Hines 233 Baseline Rd, Lavern 91750 which is a 30 day temporary
In chapter 11 through 15, afew things happen. Such as, Miss Dubose passes away after having Jem read to her for a month. In chapter 12, Calpurnia ends up taking Jem and Scout to the church attends, where we find out that she has children of her own, has a long history with the Finch’s, and is older then Atticus. During chapter 13, Aunt Alexandra ends up showing up at the Finch’s home, stating that she will stay so Scout can learn to be more ladylike. In the beginning of chapter 14, Dil returns to Maycomb after his new father chained him up to a radiator.
