To begin with, the welfare programs started in Japan when the country turned into a modern state during the Meiji period (1868-1912), which was expanded by post WW II two administrations. But in 1960s and 1970s, Japan experienced a rapid welfare growth which put Japan in same in its welfare spending with other OECD countries. Japan’s welfare state is one of the most dynamic and effective system till economic bubble burst. Not only that, it affected the welfare system and its policies all the way to its core itself. In current situation the Japanese government will start analysing large amounts of data on patient’s medical expenses and by 2015 to find ways to trim the nation’s medical expenses. Basically in Japan, a big portion of welfare budget is spent on elderly benefits. For example, statistics from 1960-2010 the medical expenses in Japan changed to minimum from 20 trillion yen, which expected to rise to 59 trillion yen by the year of 2025. For the current reform structure, Japanese government revisited patient’s first visit 120 yen to 2820 yen and additional visit by 30 yen to 720 yen. The increasing of foreigner nursing personal in Japan created a unique situation. Mainly from Indonesia, Thailand, Malaysia and even Srilankan nurses are migrating to Japanese nursing care organization, at least the trend is like this. Because of high bar level if qualification and Japanese language proficiency is a tough to handle for many. But the numbers of recognized foreign nurses are increasing over last 10 -15 years. So thus the nursing care facilities are becoming much important in the medical care system for elderly in Japan.
