4.1. Brief Synopsis: Worker 1 (W1, AFSAS Person 1) was participating in a flight-level exercise when she approached a HMMWV (OBJ1, AFSAS Object 1) and attempted to challenge and apprehend the driver. As WKR1 approached on the rear driver’s side, the driver began to leave subsequent WKR1’s foot was crushed resulting in GTFA. 4.2. Sequence of Events: WKR1 was participating in a flight level exercise where she was one of the few survivors after an attack. While searching for the intruders, WKR1 spotted intruders entering a HMMWV. WKR1 then tactilely approached the rear of the vehicle on the driver’s side, hugging the vehicle in an attempted to challenge and apprehend the intruders. When WKR1 slightly in front of the driver’s side rear tire, the intruders began to drive off, rolling over WRK1's foot. WKR1 immediately informed her flight leadership of her mishap and the exercise was terminated. WRK1 was then transported to a nearby medical facility where she was diagnosed with sever bruising and swelling of the right foot and received a medical boot and medication. On 7/20/2015 S/O EMT Perez was dispatched to MG-601 regarding a fall. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/ O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor.
Upon discharge, she returned home with her son, Michael and Viva Hospice Service. During hospice admission at home, Ms. McGinnis fell out of hospital bed and got her head stuck in the bedside commode railing. The hospice nurse, Karen called EMS and she was transported
Officers learned that the silver Subaru was the vehicle that struck Agnes. Agnes was concsious and alert when I arrived on scene. I observed a facial laceration in the area of Agnes ' forehead which was actively bleeding and formed a small puddle of blood on the roadway. There was an individual on scene prior to police arrival who placed a rag on Agnes ' head and held pressure on the wound until an ambulance arrived. When Shenandoah Ambulance arrived on scene they assessed Agnes ' injuries.
I spoke with Michelle Chilies-Kusiolek, Chief (RMO). Ms. Chilies Kusiolek declined to give age, race, color or gender. She stated that she is aware of the incident and the alleged racial slur. She contacted the Human Resource department to give her guidance on what would be the appropriate course of action.
After a college student went missing, they found him alive and lying face down and bloody by some active train tracks. The investigators of criminal law are trying to understand what led up to the incident. James Hubert, 24, had been discovered without his shoes and a torn dress shirt. When they transported him to the Grady Hospital in Atlanta, the physicians started to treat him for broken ribs, facial bruising and brain hemorrhaging. He also had the potential for paralysis.
Evans and I were able to get Cage out of the rear seat and we lifted her onto an HMH gurney. On March 7, 1998, 55 year old Mrs. Jadine Russell and her daughter were involved in a minor automobile accident. While on the side of the road assessing the damage, Jadine, her daughter, and two police officers, were taken by surprise, when alleged drunk driver, Keith Cook, lost control of his pickup hitting Jadine’s car. The vehicles then plowed into the four victims as they stood there. “All were hurt, but Jadine suffered the most severe injury when the car pinned her against a fence and ruptured her spleen, causing massive internal bleeding and leaking blood into her abdomen” (Baxtrom). Jadine, in need of serious medical attention, refused a blood transfusion.
On 10-14-2015, at approximately 1600 hours, I was notified by Sgt. E. Kelley, # 2154, of a traffic crash involving life threatening injuries. on Lumsden Road, east of Providence Road. The crash was reported via 911 at 1456 hours.
On Oct 02, 2015, cadet Dawkins approached me (Sgt VanDyke), Sgt Cooper and Sgt Bradley as we sat in Sgt Bradley’s office. He asked if he could talk to us. After responding yes to his question, cadet Dawkins entered into Sgt’s Bradleys office and closed the door. Cadet Dawkins proceeded to say that he wanted to seek medical attention for his hamstring in which he injured in the second week of the academy.
Before I even step foot in the door I could hear sobbing and the distress in a woman 's voice. As EMS personnel we took over for the cops, we placed a LUCAS device on him, it 's a device that does compressions for you.
