Critical Reflection: Provider The site I observed at was Nazareth senior living center. The facility provides short term and long term care to older adults. The health care staff working at the facility includes: Nurses, social worker, dietitian, physical therapist, occupational therapist, speech therapist recreation therapist and a part time physician. The care provider I interviewed was the social worker. When asked about her role in the care team, the social worker stated that she was an advocate for the patients. In addition, the social worker coordinates care meetings with other professions and the families. Finally, she creates discharge and long term care plans for the residents. For a patient who has a short term stay, the social worker …show more content…
At this facility the social worker spends most of their time with the patients, which is different than I had previously assumed. In previous IPE classes when we have learned about the role of the social worker, it was portrayed as a behind the scenes job. They described the social worker as spending time with the patient, and filling out paperwork for insurance companies. At Nazareth, they have designated staff that are in charge of evaluations and assessments. These individuals complete pre and post assessments, along with submitting the paperwork to Medicare or Medicaid. As a result, the social worker has more time to spend with the patient, instead of completing this type of work. A difference I noticed was that the role of the social worker in Nazareth included ordering equipment for the patients’ homes. At my last sight, Ranken Jordan, this was the role of the occupational therapist (OT). A common task for an OT is completing a home evaluation, and ordering adaptive equipment that is needed. I found it interesting that the social worker was in charge of this type of …show more content…
In the future, I will remember to provide a holistic care to the patient. This includes their physical injuries along with their spiritual and mental wellbeing. I will provide care that is patient-centered. As in the other IPE classes, patient-centered care is an essential aspect of interprofessional teamwork. It is vital to keep the priority on the patient’s wellbeing. Lastly, I will help the patient understand medical vocabulary and terms by asking questions, eliciting responses and asking the patient to repeat the information that was

Social workers have several responsibilities. They have to provide service, justice, and dignity to a client. They have to possess integrity, competence, and patience. Social workers need to possess knowledge of human rights, and how to perform scientific inquiry. Social workers occasionally have cases in which problems ensue and a solution is not found within a certain time frame.
She further discusses that social workers make sure that the patients and their families get unconditional love and support, thus maintain a positive
Agencies Final Written Reflection: Mayfair Care Center Cole and I volunteered at the Mayfair Care Center located near Heritage Park in Calgary. Being one of the older care centers in Calgary at 43 years old, the building was renovated recently and appeared to be quite new, creating an enjoyable environment during my volunteering. Each day had a schedule that Cole and I proceeded with. We would complete two activities, we always would play card bingo and then the second activity rotated week by week through the following games: balloon badminton, carpet bowling, bocci ball, shrimp throw, archery). When we arrived at the center, around 1:45-12:00 we would begin moving the residents to the first activity: card bingo.
Description: In week 7 we had visited Wesburn Manor, the Long-term Care (Clinical Placement) setting. It was our first time there, therefore as a group, we oriented the place. At this time, we went to each nursing station on each floor and introduced ourselves from the organization we were from and how we will be providing patient care to the clients in this setting. We were educated by our clinical instructor on the different access codes in the building, the policies and guidelines, our assigned floor and the appointed personal support worker. Our role of professionalism as a nursing student was represented as we came prepared and greeted each healthcare and staff member.

First of all, I had the opportunity to interview Kim Bartells who’s a Licensed Social Worker (LSW) in Michealsen Health Center and learn more about her role as a social work. Before I started interviewing Kim, I asked her if it was alright with her if I recorded the conversation and she said it was fine with it. I started the interview with asking what type of population Michealsen Health Center serves and she told me it was mostly elderly people. Kim works in a “Microlevel intervention involves working with individuals--- separately, in families, or in small groups---to facilitate change in individual behavior or in relationship” (DuBois and Miley 69). In the social work profession, it is a requirement to have a thorough understanding of ethics and some of the ethical dilemmas that one can face. On my journey as a social work student one of my assignments was specific to ethical dilemmas. The ethical dilemma that I chose to write in this paper about was multiple relationships and violating counselor client boundaries. The class that I wrote this in was HMS 102 which was values of human services. The main topic of the paper focused on the negative effects of sexual relationships.
