Currently, healthcare has shifted from curative care to preventive care. The American public benefits from preventive care because it is cost effective and has lasting impacts on the community. The initiative of Healthy People 2020 is focusing on the health and preventing disease in local communities. The goals of Healthy People 2020 are to promote healthy behaviors, encourage quality of life while eliminating disparities among all age groups within the community (Healthy People 2020). Healthy People were first started in 1979 as result from the recommendations of health awareness authorities that our nation should engage in disease prevention (Healthy People, 2020). In this discussion, an objective from the Healthy People 2020 as well as the reasons for its relevancy will be examined. The Healthy People 2020 objective I chose to focus on is heart disease and the impact it has on my local community. Heart disease is one of the leading causes of death in the United States (Centers for Disease Control and Prevention, 2014). In Franklin County, heart disease is also the leading cause of death with an annual death rate of 1,917 from 2004-2006 (The Ohio Department of Health, 2008). Education is the key in decreasing the risk factors for heart disease in general public. Cardiovascular disease accounts for one in three deaths annually n the United States (Luckhaupt & Calvert, 2014). Therefore, community nurses need to take the extra step and travel to local businesses, schools and facilities to do teaching and motivate the community for healthier lifestyles. A sitting based approach is more likely to be effective then asking an individual to take responsibility for their own health because a setting based approach allows for Joint achievement between the action areas for example if the local pool owner of Arnhem supported the “healthy heart is a better start program” by reducing prices for students of Shepherdson college, students to be able to access this facility at a cheaper price and therefore strengthening community action and enabling potential prevention of cardio vascular disease. Although there is a possibly of some students not partaking in this sitting based approach to health, the program involves many effective health promotion strategies which gives the students all the resources and support they desire. Healthy People 2020 The purpose of this paper is to talk about the health of my community. Throughout the paper, I will state long-term and short-term goals for two nursing diagnoses related to cardiovascular disease and lung cancer, as well as nursing interventions in an attempt to help improve the outcomes of cardiovascular disease and lung cancer in my community. According to Baldwin (2003) health care disparities are the differences in health and health care between population groups including race, socioeconomic status, age, location, gender, disability, and sexual orientation. Disparities limits the improvements of quality health care which could result in unnecessary health care expenses. Factors that are contributing to disparities within today’s society are lack of access to quality health care and the number of individuals who are uninsured. As the population continue to grow and become more diverse health care disparities will continue to increase.
