CCIB Intake received a SOC 341 regarding an incident which occurred on 5/12/16 involving client Christian (last name unknown) and client J.W. According to the reporting party (RP) Program Manager Jim Kaing at 3:10PM while Christian was in the kitchen eating a snack he suddenly rose from the kitchen table and walked into the "Art Room" and pule the hair of client J.W. According to the RP Christian pulled the hair with two hands resulting in some hair loss and a pin sized blood spot on the her scalp. Brenda prompted Christian to have seat and the RP requested Christian to apologized to client J.W (which is consistent with his mother 's request due to his understanding of the nature of his behavior). Once he apologized Christian attempted to scratch
COMPLAINT This matter was opened by bar counsel on January 25, 2017. In or about December 2016, while preparing for hearing on B.B.O. File Nos. C5-14-0055 and C5-16-0008, bar counsel learned that the respondent, Laura Marshard, an assistant district attorney in the Cape and Islands District Attorney’s Office, had allegedly provided information concerning an ongoing police investigation to the target of the investigation. Bar counsel also learned that Marshard allegedly violated the Rules of Professional Conduct while handling a criminal complaint.
CCIB LPA Perryman-French received a call from Mildren. her husband Julian lives in this facility. He is non-ambulatory and requires assistance to utilize the bathroom. Mildred stated that when staff call in, the director does not replace them or cover behind them, the result is that her husband cannot get the assistance he needs to use the bathroom. This has increased his accidents.
CCIB received a SOC 341 from APS for the following residents: Daniel Hourihan (DOB: 10/03/54), Gerardo Guerrero (DOB: 01/02/95) and Gerald Gaither (DOB: 06/14/59). Per the reporting party, Lee Nln, the provider/Owner Cheryl Oliver has been advertising her independent living facility as a board and care. RP stated that her clients have been paying the board and care rate and the client require medication management. RP stated that in April 2018, client Daniel Hourihan moved to another facility. The rent for Daniel was sent to Ms. Oliver who continued to cash the rent checks for Daniel.
CCIB Intake received a cross report call from Marisa Romero with the County of San Diego CPS. Marisa stated she met with Detective Elizabeth Hundley to initiate the investigation of Gabriel Munoz DOB: 4/6/15 head injury. Marisa stated she and the Detective have statements from teachers regarding the incident. The Director was also interviewed and denied the incident happened. The Director continued to deny the incident even when the teacher Monika Huynh admitted the child hit his head when she fell.
Circumstance: Ms. Smalls (MHP), Mrs. Gailliard (MHS), Clarence and Ms. Elizabeth Strong (DSS Worker) schedule medical appointment with the MUSC Foster Care Clinic. Action: MHP called Tara Peevy, RN at the MUSC Foster Care Clinic after MHS explained leaving several messages. Ms. Strong explain emailing the referral form to the clinic. Machelle Green explain receieing the referral form, however unable to reach the DSS worker for additional infromaiton.
Predication: On 11/11/17, Asset Protection Manager (APM) Kristin Catucci contacted APM Jakub Orlando regarding Customer Service Associate (CSA) Anthony Stoddart who was suspected of taking money out of the register for personal benefit. Facts: On 11/14/17, APM Orlando reviewed CCTV footage along with POS electronic journal to confirm this allegation. CCTV footage reviled that CSA Stoddart took money from the bottom of the register and placed it into his pocket.
C: Mrs. Mack (MHS) reported Jaheim Brown display physical and verbal aggression. A: At 9:58 PM Ms. Smalls (on call case coordinator) was notified by Mrs. Mack that Jaheim started throwing things around the home, refusing to comply with requests and refuse to communicate with Mrs. Mack. Mrs. Mack called law enforcement after Jaheim broke the bedroom closet door and other personal property in the bedroom.

Juarez, MSW, CSAI, Department of Children and Family Services (626) 569-6929, (213) 760-2590 cell who had questions regarding referral numbers 0698-6622-2040-4072794 and 1410-9736-5976-4064898 dated 3/23/16. The referrals were not received or processed, however you completed a Case Management Visit on 4/1/16 regarding information contained on the referrals. How would you like me to handle the referrals. I can attach a LIC812 on your LIC809 if you prefer. I don’t believe I need to process a complaint due to your case management visit.
On 10/24/2015, CM met with the client to complete Bi-Weekly ILP Review. Client was able to come to the 3rd floor social service. In the meeting client was dressed in black floral skirt and beige sweeter and jacket. Client appears to be happy she was humming. She appears to be cooperative and friendly.
Kevin Moore, an inmate formerly housed at the Poplar Hill Pre-Release Unit, raises a failure to protect claim against Defendants, Sergeant William D. Jones and Correctional Officer II Ivan Tilghman, resulting from a physical altercation Plaintiff had with another inmate. He further alleges that he would not have sustained injury if Defendant Warden John Wolfe had appropriate security in place but fails to allege any allegations against him. Irrespective of whether Plaintiff has stated a claim, he failed to exhaust administrative remedies, prior to filing suit; therefore, the Amended Complaint must be dismissed. ECI is not a person amenable to suit and is entitled to sovereign immunity.
CCIB Intake received a SOC 341 from reporting party Sean Ahern regarding Chris Rueda and Katler Paige. According to the reporting party on 3/5/17 at 10:15am Paige became upset after speaking with her mother and began screaming at Chris attempting to get into his room. Paige threatened she was going to write "fuck you" on his door. He opened the door and Paige grabbed his hand digging her fingernails into his skin. Chris pulled away causing Paige to scratch him on his left hand resulting in an inch laceration along with another smaller scratch.
