Standard 9.04 a defines raw test data and scaled scores as raw information disclosed by clients during examination. Test data are client’s response to test and scaled scores and these information’s have not been released or subjected to analyzing by a third party. The scaled score are the psychologist note during the testing and does not include psychotherapy notes. The revised rules for test data was implemented in 2003 and new rules were added to help client have autonomy over their raw data.

During Q1-Q2 2015, I have achieve 5 out of 6 KPI target. The numbers provided below are average KPI from January to June 2015. Posting against PO can be maintained by not forgetting to deduct WHT and recording the correct amount of VAT.

Unit 4 is the preparation of treatment in which the baseline data (activity level) is gathered. This includes daily activity record and behaviour contract. A daily activity record ( hour by hour ) is used to monitor activities clients is already doing, to see the frequency of healthy and unhealthy behaviours. (Lejuez, 2001, p. 265). In this phase, the client is encouraged to create an environment, one that promotes healthy behaviours (Lejuez, 2001).
On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment. On 11/27/16 CM went to conduct the Safe Sleep inspection with the DHS Compliance Analyst Jeanette Chirico, while in the second floor we went to unit #222 assigned to Ms. Tiana Footman and her three children.

A local GP should be able to recognise a service users symptoms and diagnose the service users symptoms of mental health or psychosis then they should refer the service user to an intervention service were they will then be assessed. When dealing with substances, they will have a potential hazard which can affect the health care setting. The substance can be a potential hazard in the residential care home as there is clinical waste and cleaning products. As the nurse and the staff member will have to make sure that the cleaning products is locked in a room so that the visitors and the residents won 't be able to go in and touch the chemical products. There is a law called COSHH Regulation 2005, which stand for Control Of Substance Hazardous to Health regulation 2005. COSHH is when the hazardous substance have to be handled in a right way and it has to be stored in a minimise risk which is present.
At the meet last night, Bella wasn 't feeling well and I kept her home from school today due to symptoms of stomach flu. It 's looking as though she will miss school again tomorrow, so she will also miss practice. If the symptoms subside and she starts to feel better she will be there.
